某院中药饮片处方药味调查及影响因素分析

摘　　要：目的通过调查某院中药饮片处方药味,分析影响药味的因素,指导临床合理用药。方法收集某院2017年3月—2018年3月门诊内服中药饮片处方,每月随机抽取100张,共1200张进行点评。结果调查1200张处方中,男性患者522人,女性患者678人,平均年龄(51.48±16.58)岁,平均一张处方药味(16.36±4.07)味。不同性别、年龄、医师职称、就诊科室药味数均值比较,差异有统计学意义(P<0.05)。在60张调剂差错处方中,药味数在20以上处方差错概率最高为11.92%;67张不合理处方中药味20以上处方出现不合理概率最高为9.27%。结论患者性别差异、年龄大小,开药医生资历以及不同疾病种类对中药饮片处方药味有所影响,调剂差错的发生和不合理处方的出现频率都与中药处方药味有直接关系。少数科室中药处方药味过多的问题应引起重视,呼吁医师开方精细化调整,用药精当促进临床合理、安全使用中药饮片。Objective To investigate the prescription of traditional Chinese medicine in a hospital,to analyze the factors affecting the herbal number,and to guide the clinical rational use of drugs.Methods From March 2017 to March 2018,100 Chinese medicine decoction prescription were collected randomly every month,and a total of 1200 were reviewed.Results Among the 1200 prescriptions,522 were male patients and 678 were female patients.The average age was(51.48±16.58)years old,and the average taste of a prescription was(16.36±4.07).The difference of the mean value of drug taste in different genders,ages,physician titles and medical departments were significant,P<0.05.Among the 60 prescription errors,the error probability of medicine taste number above 20 was 11.92%.The highest probability of the 67 unreasonable prescriptions above 20 was 9.27%.Conclusion Patients'gender difference,age,prescribing doctor's qualification and different kinds of diseases have an impact on herbal number of Chinese medicine decoction prescription.The occurrence of dispensing errors and the frequency of unreasonable prescriptions are all directly related to herbal number of Chinese medicine decoction prescription.The problem of too much traditional Chinese medicine in a few departments should be paid attention to,and physicians are called on to make prescription refined and adjust,so as to promote the rational and safe use of traditional Chinese medicine in clinical practice.
