益气利水方隔药灸治疗慢性肾功能衰竭

摘　　要：目的:观察益气利水方隔药灸联合西医综合治疗脾肾阳虚型慢性肾功能衰竭的临床疗效,并观察其对患者生活质量的改善程度。方法:80例脾肾阳虚型慢性肾功能衰竭患者随机平均分为观察组和治疗组。对照组给予西医综合治疗,治疗组在对照组治疗基础上采用益气利水方隔药灸疗法,两组疗程均为2个月。比较两组患者治疗前后症状体征积分、生活质量评分,血肌酐(serum creatinine,Scr)、血尿素氮(blood urea nitrogen,BUN)、24小时尿β2微球蛋白(β2-microglobulin,β2-MG)、内生肌酐清除率(creatinine clearance,Ccr)、红细胞计数(red blood cell,RBC)、血红蛋白(hemoglobin,Hb)水平。结果:治疗组有效率(70.0%)明显优于对照组(42.5%),差异有统计学意义(P<0.01)。治疗后,治疗组各种症状均有显著改善,改善率均高于70%,与对照组比较,差异均有统计学意义(P<0.05)。治疗组治疗后的Scr、BUN以及24小时尿β2-MG明显下降(P<0.05),Ccr明显升高(P<0.05),RBC明显升高(P<0.05);以上指标与对照组治疗后比较,差异均有统计学意义(P<0.05)。治疗组治疗后的生活质量评分显著高于治疗前(P<0.05),且显著高于对照组治疗后(P<0.05)。结论:益气利水方隔药灸联合西医治疗脾肾阳虚型慢性肾功能衰竭疗效显著,可有效改善患者肾功能,提高患者生活质量。Objective:The study is to research the clinical efficacy of herb-partitioned moxibustion of Yiqi Lishui Prescription combined with Western medicine in the treatment of chronic renal failure(CRF)of Spleen-Kidney Yang deficiency patients and observe the improvement patient′s life quality.Method:80 spleen-kidney Yang deficiency patients with CRF were randomly divided into control group and treatment group.Both groups were given comprehensive western medicine treatment,however,the treatment group was added with Herb-partitioned Moxibustion.The treatment course of both groups was 2 months.Compare the scores of symptoms and signs,quality of life scores,serum creatinine(Scr),blood urea nitrogen(BUN),24-hour urineβ2-microglobulin(β2-MG),Endogenous creatinine clearance(Ccr),red blood cell(RBC),and hemoglobin(Hb)level before and after the treatment of the two groups.Results:The effective rate(70.0%)of the treatment group was significantly higher than the control group(42.5%),and the difference was statistically significant(P<0.01).After treatment,various symptoms in the treatment group were notably improved,and the improvement rate was higher than 70%.The difference was statistically significant compared with the control group(P<0.05).After Treatment,Scr,BUN,and 24-hour urineβ2-MG decreased significantly(P<0.05),Ccr increased(P<0.05),and RBC increased(P<0.05)in the treatment group.The above indicators were compared with the control group after treatment,and the differences were statistically significant(P<0.05).Also,The quality of life score of the treatment group was higher than that before treatment(P<0.05)and was significantly higher than that of the control group after treatment(P<0.05).Conclusion:The Herb-partitioned Moxibustion of Yiqi Lishui Prescription combined with Western medicine has a significant effect on the treatment of CRF of Spleen-Kidney Yang deficiency Patients,which can effectively improve the indicators of renal function in CRF patients,and it is favorable for improving patients life quality.
