摘  要:目的从供给侧角度分析医疗集团化发展存在的问题及原因,为医疗集团化模式的完善和推广提供案例。方法以温州市某医疗集团260名医务人员为研究对象开展问卷调查,描述医务人员对医疗集团化改革的认知及工作体验,比较牵头医院和成员单位医务人员对集团化运行效果的评价。结果牵头医院和成员单位医务人员对医疗集团化改革的认知较为一致,主要了解途径为医疗卫生系统宣传;牵头单位不同年龄段的医务人员对医疗集团化总体效果评价有差异(p=0.009),成员单位医务人员对医疗集团的认识程度不同与总体效果评价有差异(p=0.006);牵头医院医务人员大部分赞同半紧密型合作,成员单位医务人员较多赞同半紧密型和完全紧密型,差异具有统计学意义(p<0.001)。结论医务人员对医疗集团化改革认识程度较好,但效果评价存在不一致,双向转诊效果不理想,人才激励机制、信息系统建设等方面还需加强。Objective To analyze the problems and causes of the collectivization development of medical group from the supply-side perspective,so as to provide examples for the improvement and promotion of the medical group model.Methods A questionnaire survey was conducted on 260 medical staff of a medical group in Wenzhou.Medical staff’s cognition and working experience of medical group reform.The evaluation of the operation effect of medical group from the medical staff of the leading hospital and member hospitals were compared.Results The medical staff of the leading hospital and member units had consistent cognition of the medical group reform,and they mainly knew about it from the propaganda of medical system.The medical staff of different age groups in the leading hospital had different evaluations of the overall effect of medical group reform(p=0.009).The medical staff in member hospitals had different levels of awareness of the medical group and different evaluations of the overall effect(p=0.006).Most of the medical staff in the leading hospital supported semi-close cooperation,and the medical staff of member hospitals mostly supported semi-close and complete close cooperation(p=0.000).Conclusion The medical staff has a good cognition of the medical group reform,but the effect evaluation is inconsistent.The twoway referral is not developing well,and the talent incentive mechanism and information system construction need to be strengthened.
