盆腔炎性疾病后遗症患者中医体质分类与中医证型相关性研究

摘　　要：目的研究盆腔炎性疾病后遗症(SPID)患者中医体质分类与中医证型的关系,为防治SPID提供新的思路。方法以230例SPID患者为观察组,230例健康体检女性为对照组,根据《中医体质分类与判定表》对入组病例进行中医体质分类,运用Logistic回归分析探讨对SPID发病有影响的中医体质类型;运用对应分析探讨SPID患者中医体质类型与中医证型之间的关系。结果平和质、阳虚质、湿热质、气郁质在2组间分布差异显著(P<0.01);Logistic回归分析提示阳虚质、湿热质、气郁质为SPID的易感体质(P<0.05),平和质是SPID的保护体质(P<0.05);对应分析提示阳虚质较其他体质人群更容易出现寒湿瘀阻证,气郁质人群更容易出现气滞血瘀证,湿热质人群更容易出现湿热瘀结证,阴虚质更容易出现血瘀肾虚证。结论SPID与中医体质类型有密切的关系,阳虚质、湿热质、气郁质为SPID的易感体质,平和质是保护体质;SPID患者中医体质类型与中医证型存在对应关系。Objective(Department of Gynecology,Puning Hospital of Traditional Chinese Medicine,Guangdong Province,Puning 515300,China)Methods Taking 230 SPID patients as the observation group and 230 healthy females as the control group,the Chinese patients were classified according to the Traditional Chinese Constitution Classification and Judgment Table.Logistic regression analysis was used to explore the types of TCM constitution that have an effect on the incidence of SPID Correspondence analysis was used to explore the relationship between TCM constitution type and TCM syndrome type in SPID patients.Results The distribution of Pinghe quality,Yang deficiency quality,damp heat quality,and qi depression quality was significantly different between the two groups(P<0.01).Logistic regression analysis showed that Yang deficiency quality,damp heat quality,and qi depression quality were SPID susceptible constitutions(P<0.05),peace is the SPID’s protective constitution(P<0.05).Correspondence analysis shows that yang deficiency is more prone to cold dampness and stasis syndrome than other constitutional populations,qi stagnation population is more likely to develop qi stagnation blood stasis syndrome,and dampness-heat quality dampness-heat stasis syndrome is more likely to occur,and yin deficiency is more likely to cause blood stasis and kidney deficiency syndrome.Conclusion SPID is closely related to the type of Chinese medicine constitution.Yang deficiency,dampness-heat,and qi depression are the susceptible constitutions of SPID.Peaceful constitution is the protection constitution.There is a corresponding relationship between the type of Chinese medicine constitution and the type of TCM syndrome in SPID patients.
