血清CX3CL1、Lp(a)与冠心病病变严重程度的相关

摘　　要：目的探讨血清趋化因子CX3CL1、脂蛋白a[Lp(a)]与冠心病(CHD)病变严重程度的相关性。方法选取2017年9月~2020年5月我院收治的CHD患者80例作为CHD组,根据冠脉造影结果分为Ⅰ亚组(n=16)、Ⅱ亚组(n=22)、Ⅲ亚组(n=27)、Ⅳ亚组(n=15),另选取80例择期健康体检者作为健康对照组,比较两组临床基本特征及CX3CL1水平、Lp(a)水平,分析CHD不同冠脉病变严重程度患者血清CX3CL1、Lp(a)水平及血清CX3CL1、Lp(a)与CHD危险因素的相关性。结果CHD组与健康对照组年龄、性别、BMI、收缩压、舒张压、TC、LDL-C、HDL-C比较,差异无统计学意义(P>0.05);CHD组合并代谢综合征、高脂血症、糖尿病的比例和TG、hs-CRP水平高于对照组,差异有统计学意义(P<0.05)。CHD组血清CX3CL1、Lp(a)水平高于对照组,差异有统计学意义(P<0.05)。CHD组中Ⅰ亚组、Ⅱ亚组血清CX3CL1水平比较,差异无统计学意义(P>0.05);Ⅲ亚组、Ⅳ亚组血清CX3CL1水平高于Ⅰ亚组,Ⅲ亚组血清CX3CL1水平高于Ⅱ亚组,差异有统计学意义(P<0.05)。Ⅱ亚组、Ⅳ亚组血清Lp(a)水平高于Ⅰ亚组,差异有统计学意义(P<0.05)。血清CX3CL1水平与CHD各危险因素均无相关性(P>0.05),血清Lp(a)水平与hs-CRP呈正相关(r=0.277,P<0.05),而与其它危险因素无相关性(P>0.05)。结论血清CX3CL1、Lp(a)在CHD患者中表达水平升高,但无法预测CHD病变严重程度。Objective To explore the correlation between serum chemokine CX3CL1,lipoprotein a[Lp(a)]and the severity of coronary heart disease(CHD).Methods Selecting 80 CHD patients admitted to our hospital from September 2017 to May 2020 as the research object,according to the results of coronary angiography,it was divided into group I(n=16),group II(n=22),group III(n=27),group IV(n=15).At the same time,80 patients with elective healthy physical examination were selected as the healthy control group.The basic clinical characteristics,CX3CL1 level and Lp(a)level of the two groups were compared,and the serum CX3CL1,Lp(a)levels and serum CX3CL1 levels of patients with different coronary artery disease severity of CHD were analyzed.Correlation between Lp(a)and CHD risk factors.Results There was no significant difference between CHD group and healthy control group in age,sex,BMI,systolic blood pressure,diastolic blood pressure,TC,LDL-C,HDL-C(P>0.05);The proportion of CHD combined with metabolic syndrome,hyperlipidemia,diabetes and the levels of TG and hs-CRP were significantly higher than those of the control group,the difference was statistically significant(P<0.05);Serum CX3CL1 and Lp(a)levels in the CHD group were significantly higher than those in the control group,the difference was statistically significant(P<0.05);There was no significant difference in serum CX3CL1 levels between groups I and II(P>0.05);The levels of serum CX3CL1 in groups III and IV were significantly higher than those in group I,the levels of serum CX3CL1 in group III were significantly higher than those in group II,the difference was statistically significant(P<0.05).The level of serum Lp(a)in groupsⅡandⅣwas higher than that in groupⅠ,the difference was statistically significant(P<0.05).Serum CX3CL1 level was not correlated with any risk factors of CHD(P>0.05),serum Lp(a)level was positively correlated with hs-CRP(r=0.277,P<0.05),but not correlated with other risk factors(P>0.05).Conclusion The expression level of serum CX3CL1 and Lp(a)in CHD pat
