中医定向透药治疗对颅脑损伤患者炎症状态、气管套管停留时间和肺功能的影响

摘　　要：目的:探讨中医定向透药治疗对颅脑损伤患者炎症状态、气管套管停留时间和肺功能的影响。方法:选取2019年4月~2020年4月收治的颅脑损伤患者120例,分为对照组和观察组各60例。两组患者入院后均给予常规治疗(手术、气管套管有创机械通气等),待患者病情稳定后,观察组给予中医定向透药治疗。比较治疗前后两组白细胞计数、白细胞介素-6、降钙素原、C反应蛋白、动脉血氧分压、动脉血二氧化碳分压、气管套管停留时间和住院时间,记录两组不良反应发生情况。结果:治疗后,观察组的白细胞计数及血清白细胞介素-6、降钙素原、C反应蛋白水平均低于对照组(P<0.05);观察组的动脉血氧分压高于对照组,动脉血二氧化碳分压低于对照组(P<0.05);观察组气管套管停留时间、住院时间均短于对照组(P<0.05)。两组治疗期间未见严重不良反应及过敏现象。结论:在常规治疗的基础上,辅以中医定向透药治疗有利于减轻颅脑损伤患者的炎症反应,改善其肺功能,缩短气管套管停留时间和住院时间,增加临床获益。Objective:To investigate the effects of Traditional Chinese Medicine directional transdermal therapy on inflammatory status,tracheal cannula indwelling time and lung function of patients with craniocerebral injury.Methods:From April 2019 to April 2020,120 patients with craniocerebral injury were selected and divided into control group and observation group,with 60 cases in each group.Two groups'patients were given conventional treatment after admission(surgery,tracheal cannula invasive mechanical ventilation,etc.),after the patient's condition was stable,the observation group was given Traditional Chinese Medicine directional transdermal therapy.Before and after treatment,the leukocyte count,interleukin-6,procalcitonin,C-reactive protein,arterial partial pressure of oxygen,arterial partial pressure of carbon dioxide,tracheal intubation stay time and hospital stay were compared between the two groups.Recorded the occurrence of adverse reactions in the two groups.Results:After treatment,the leukocyte count and levels of serum interleukin-6,procalcitonin and C-reactive protein in the observation group were lower than those in the control group(P<0.05);the arterial partial pressure of oxygen in the observation group was higher than that in the control group,and the arterial partial pressure of carbon dioxide in the observation group was lower than that in the control group(P<0.05);the tracheal cannula indwelling time and hospitalization time in the observation group were shorter than those in the control group(P<0.05).There were no serious adverse reactions and allergic manifestations during treatment in the two groups.Conclusion:On the basis of conventional treatment,combined with Traditional Chinese Medicine directional transdermal therapy is beneficial to relieve the inflammatory status of patients with craniocerebral injury,improve their lung function,shorten the tracheal cannula indwelling time and hospitalization time,and increase clinical benefits.
