加味阳和汤联合手术治疗反复发作破溃巨大痛风石的临床效果

摘　　要：目的观察加味阳和汤联合手术治疗反复发作破溃巨大痛风石的临床效果。方法选择2018年6月至2019年7月西安市第五医院收治的32例痛风石反复发作破溃的痛风性关节炎患者,根据治疗方式将其分为对照组和研究组,每组16例。对照组给予痛风石切除清创术,研究组在对照组的基础上术后口服加味阳和汤治疗。比较两组的治疗效果。结果研究组的治疗优良率高于对照组,复发率低于对照组(P<0.05)。研究组的伤口愈合时间短于对照组,关节功能恢复占比高于对照组(P<0.05)。术后7 d,两组的血清ESR、CRP、PCT水平及WBC均较治疗前降低,且研究组低于对照组(P<0.05)。术后2、4周,两组血UA水平均较术前1 d显著降低,且研究组低于对照组(P<0.05)。结论加味阳和汤联合手术治疗反复发作破溃巨大痛风石具有确切的效果,可促进患者术后伤口愈合及关节功能恢复,减轻炎性反应,降低血UA水平及复发率。Objectire To observe the elinical effect of modified Yanghe decoction combined with openation in the treatment of recurrent ulcerated giant gout stone.Methods From June 2018 to July 2019,32 cases of gouty arthritis:patients with recunent ulceauted giant gout stone admiued in Xi'an No.5 hospital were selected and divided into control group and study group according to the treatment,with 16 cases in each group.The control group was given gout stone resection debrdement,and the sudy group was given modified Yanghe decoction after operation on the basis of the control group.The therapeutie effects of the two groups were compared.Results The excellent and good rate of treatment in the study group was higher than that of the control group,and the recurence rate was lower than that of the control group(P<0.05).The wound healing time of the study group was shorter than that of the control group,and the proprtion of joint function recovery was higher than that of the control group(P<0.05).At 7 d after operation,the levels of serum ESR,CRP,PCT and WBC of the two groups were lower than those before treatmen,and those of the study group were lower than the control group(P<0.05).At 2 and 4 weeks after operation,the blood UA levels of the two groups were significantly lower than those at 1 d before operation,and those of the study group were lower than the control group(P<0.05).Conclusion The modified Yanghe decoetion combined with surgery in the treatment of recurrent ulcerated giant gout stone has definite effect,which can promote the wound healing and joint function recovery,reduce the inflammatory reaction,decrease the hlood UA level and recurrence rate.
