经颅多普勒超声联合颈部血管彩超对脑白质疏松症患者血管的评价作用

摘　　要：目的:探讨经颅多普勒超声联合颈部血管彩超对脑白质疏松症患者血管的评价作用。方法:选取于2018年3月~2019年3月本院收治的60例脑白质疏松症患者为观察组,另选择60例无脑白质疏松症患者为对照组,两组患者均进行经颅多普勒超声和颈部血管彩超检查,对比两组患者的双侧大脑中动脉(MCA)的收缩期血管流速(Vs)值,舒张期血管流速(Vd)值以及平均血流速度(Vm)值、搏动指数(PI)值、颈动脉近、远段的斑块数量。结果:观察组MCA双侧的Vs值比对照组低,PI值比对照组高,颈动脉板块数量比对照组多,差异有统计学意义(P<0.05);轻度组RMCA-Vs、LMCA-Vs>中度组>重度组,RMCA-PI、LMCA-PI及颈动脉板块数量<中度组<重度组,差异有统计学意义(P<0.05)。结论:脑白质疏松症患者采用经颅多普勒超声联合颈部血管彩超检测,可准确检测出患者的血流情况和斑块具体情况,更精准评估患者的病情严重程度。Objective:To investigate the evaluation effect of transcranial Doppler ultrasound combined with cervical vascular color Doppler ultrasound on blood vessels in patients with leukoaraiosis.Methods:60 patients with leukoaraiosis admitted to our hospital between March 2018 and March 2019 were selected as the observation group,and another 60 patients without leukoaraiosis were selected as the control group.Both groups of patients underwent transcranial surgery.Pule ultrasound and cervical vascular color Doppler ultrasound examination,comparing the bilateral middle cerebral artery(MCA)systolic vascular velocity(Vs)value,diastolic vascular velocity(Vd)value and average blood flow velocity(Vm)value of the two groups of patients,pulsatility index(PI)value,the number of plaques in the proximal and distal segments of the carotid artery.Results:The bilateral Vs value of the MCA of the observation group was lower than that of the control group,the PI value was higher than that of the control group,and the number of carotid artery plates was more than that of the control group.The difference was statistically significant(P<0.05);the mild group RMCA-Vs,LMCA-Vs>moderate group>severe group,RMCA-PI,LMCA-PI and the number of carotid artery plates<moderate group<severe group,the difference was statistically significant(P<0.05).Conclusion:The use of transcranial Doppler ultrasound combined with cervical color Doppler ultrasound in patients with leukoaraiosis can accurately detect the patient’s blood flow and plaque conditions,and more accurately assess the severity of the patient’s condition.
