尿常规检查对尿路感染的诊断价值研究

摘　　要：目的分析尿常规检查对尿路感染的诊断价值。方法34例疑似尿路感染患者作为研究组,选择同时期在本院进行体检的健康者34例作为参照组。两组均接受尿常规检查,对比两组白细胞计数、红细胞计数,并以细菌培养检验结果为金标准,分析尿常规检查的灵敏度、特异度、准确性、阴性预测值以及阳性预测值。结果研究组患者的白细胞计数(32.22±3.01)个/HP和红细胞计数(7.72±3.99)个/HP均明显高于参照组的(7.46±2.94)HP、(4.06±2.68)个/HP,差异具有统计学意义(P<0.05)。细菌培养检验结果中,阳性和阴性患者分别为14、20例,尿常规检查中白细胞、细菌计数以及亚硝酸盐诊断尿路感染的灵敏度、特异度、准确性、阴性预测值以及阳性预测值分别为35.7%、85.0%、64.7%、65.4%、62.5%,71.4%、70.0%、70.6%、77.8%、62.5%及57.1%、75.0%、67.6%、71.4%、61.5%。结论尿路感染患者应用尿常规检查可以为临床诊断提供科学、准确的参考依据,便于患者及时获得有效治疗,临床应用价值显著,值得大力推荐。Objective To analyze the diagnostic value of routine urine test in urinary tract infection.Methods There were 34 patients with suspected urinary tract infection selected as the study group,and 34 healthy subjects who underwent physical examination in our hospital during the same period as the reference group.Both groups received routine urine tests.The white blood cell count and red blood cell count were compared between the two groups,and the bacterial culture test results were used as the gold standard to analyze the sensitivity,specificity,accuracy,negative predictive value and positive predictive value of routine urine tests.Results The white blood cell count(32.22±3.01)HP and red blood cell count(7.72±3.99)HP of the research group were obviously higher than(7.46±2.94)HP and(4.06±2.68)/HP of the reference group,and the difference was statistically significant(P<0.05).In the bacterial culture test results,there were 14 positive cases and 20 negative cases respectively.The sensitivity,specificity,accuracy,negative predictive value,and positivepredictive value of white blood cell,bacterial count,and nitrite in the diagnosis of urinary tract infection in routine urine tests were 35.7%,85.0%,64.7%,65.4%,62.5%,and 71.4%,70.0%,70.6%,77.8%,62.5%and 57.1%,75.0%,67.6%,71.4%,61.5%.Conclusion Routine urine test can provide scientific and accurate reference for clinical diagnosis for patients with urinary tract infection,and facilitate patients to obtain effective treatment in time.The clinical application value is significant,which is worthy of strong recommendation.
