摘 要:目的探讨靶控输注静脉麻醉与静吸复合麻醉对腹腔镜胆囊切除术后的影响研究。方法选取2019年1月至2020年5月盐城市亭湖区人民医院行腹腔镜胆囊切除术的67例患者作为研究对象,随机数字表法分为对照组(33例)和观察组(34例)。对照组患者予以靶控输注静脉麻醉,观察组患者接受静吸复合麻醉。比较两组患者麻醉后苏醒相关指标、认知、意识情况和不良反应。结果观察组患者术后苏醒时间、拔管时间,与对照组患者比较,差异无统计学意义(P>0.05);观察组患者的自主通气恢复时间、定向力恢复时间均短于对照组(P<0.05);观察组患者术后1、3 h的认知功能评分高于对照组(P<0.05);观察组患者术后1、3 h的意识状态评分均优于对照组(P<0.05);术后1 d,两组患者认知功能评分和意识状态评分比较差异无统计学意义(P>0.05);观察组患者不良反应发生率5.88%,低于对照组12.12%,但差异无统计学意义(P>0.05)。结论静吸复合麻醉方式应用于腹腔镜胆囊切除术中,能减少对患者术后认知功能、意识状态的影响,且不良反应少,与靶控输注麻醉相比效果更好。Objective To investigate the effect of target controlled infusion intravenous anesthesia and intravenous-inhalation combined anesthesia on laparoscopic cholecystectomy.Methods A total of 67 patients who underwent laparoscopic cholecystectomy at Tinghu District People's Hospital of Yancheng from January 2019 to May 2020 were selected as study subjects,and divided into control(n=33)and observation(n=34)groups by random number table method.Patients in the control group were given total intravenous anesthesia by target controlled infusion,and patients in the observation group received intravenous-inhalation compound anesthesia.The indicators related to awakening after anesthesia,cognition,consciousness,and adverse effects were compared between the two groups.Results The postoperative awakening time and extubation time of patients in the observation group were not significantly different from those of patients in the control group(P>0.05);the recovery time of spontaneous ventilation and the recovery time of directional force in the observation group patients were shorter than in the control group(P<0.05);the cognitive function scores of patients in the observation group were higher than those of the control group at 1 and 3 h after surgery(P<0.05);the scores of the state of consciousness of the patients in the observation group were better than those of the control group at 1 and 3 h after surgery(P<0.05);at 1 day after surgery,there were no significant differences in cognitive function scores and conscious state scores between the two groups(P>0.05);the incidence of adverse effects in the observation group was 5.88%,which was lower than that in the control group 12.12%,but the difference was not statistically significant(P>0.05).Conclusion The application of intravenous-inhalation combined anesthesia in laparoscopic cholecystectomy can reduce the impact on postoperative cognitive function and consciousness of patients,with fewer adverse reactions,compared with target controlled infusion anesthesia.
