基于结果的狂犬病被动免疫制剂注射培训方法的探讨

摘　　要：目的通过基于结果的评价对医务人员注射被动免疫制剂的效果进行评估,探索伤口处理培训关键点,实现有效的培训。方法选取来自全国各地狂犬病预防门诊的28名医务人员作为研究对象,随机分为2人一组,采用考核评价量表对被动免疫制剂注射效果进行评估,并结合结果进行反馈。反馈后再次操作,并再次评估。采用解剖检查的方法对伤口周围肌肉组织中被动免疫制剂浸润情况进行评估分级,并进行一致性检验。结果在反馈前,14组学员中,A级9组,B级2组,C级2组,D级1组。反馈前得分的均值为3.36分,标准差1.01。在接受反馈后,14组学员的评级均为A级,得分的均值为4.00,标准差0.00。P=0.03<0.05,=2.386,差异具有统计学意义。结论直观的反馈机制是培训的关键点。基于结果的狂犬病被动免疫制剂注射培训可使学员掌握标准、规范的被动免疫制剂注射方法。Objective Through the outcome-based evaluation,the infiltration status of surrounding tissues after the in-jection of passive immune preparations by clinic staff was evaluated,and the key points of wound treatment trainingwere explored to achieve effective training.Methods 28 medical staff from the rabies prevention clinics across thecountry as trainees were selected as subjects,and the evaluation of the effectiveness of passive immunization prepara-tions by using the assessment scale was performed.Feedback was given according to the results.Then trainees repeatedthe injection procedure and the infiltration status was evaluated again.The anatomical examination method was used toevaluate the infiltration of the muscle tissue around the wound,and the consistency test was performed.Results Beforethe feedback,among the 14 groups of participants,9 groups at level A,2 groups at level B,2 groups at level C,and 1group at level D.The average score before feedback was 3.36 and the standard deviation was 1.01.After the feedback.all the 14 groups of participants were at level A,with average score of 4.00 and standard deviation of 0.00.P=0.03<0.05,1=2.386.The difference is statistically significant Conclusion The direct feedback mechanism is the key point oftraining.Outcome-based training of rabies passive immunization injection can enable trainces to master standard pas-sive immunization injection methods.
