摘  要:目的:观察艾灸"犊鼻""足三里"穴对膝骨关节炎(KOA)模型兔膝关节软骨病理学及行为学的影响,为艾灸的局部抗炎、镇痛作用提供依据。方法:将40只健康新西兰大耳白兔按随机数字表法分为对照组、模型组、艾灸组、抑制剂组,每组10只。模型组、艾灸组、抑制剂组兔采用注射木瓜蛋白酶法造模,造模成功后,艾灸组、抑制剂组实施干预治疗。治疗结束后,比较各组KOA模型兔HE染色后软骨病理学改变和Lequesne评分。结果:与模型组比较,艾灸组、抑制剂组炎症细胞聚集现象明显减轻,艾灸组与抑制剂组炎症细胞聚集现象差异不明显;艾灸组、抑制剂组治疗1周、2周、3周后Lequesne评分均明显低于模型组(P<0.01);艾灸组与抑制剂组Lequesne评分比较,差异无统计学意义(P>0.05);艾灸组、抑制剂组治疗1周、2周、3周后Lequesne评分均明显低于治疗前(P<0.01),模型组仅于治疗第3周后Lequesne评分低于治疗前(P<0.01)。结论:艾灸与p38抑制剂SB203580均能降低KOA兔膝关节软骨细胞炎症聚集,减轻膝关节局部肿胀、疼痛,改善其活动步态与关节活动度,提示p38MAPK信号通路可能参与了艾灸治疗膝骨关节炎的过程。Objective: To observe the effect of moxibustion on the pathology and behavior of knee joint cartilage in rabbits with knee osteoarthritis(KOA) at "Dubi"(ST35) and "Zusanli"(ST36) points, so as to provide evidence for the local anti-inflammatory and analgesic effects of moxibustion. Methods: A total of 40 healthy New Zealand white rabbits were divided into control group, model group, moxibustion group, and inhibitor group according to a random number table, with 10 rabbits in each group. Rabbits in the model group, moxibustion group and inhibitor group were modeled by papain injection. After the model was successfully built, the moxibustion group and inhibitor group were treated with intervention. After the treatment, the pathological changes of cartilage and Lequesne score after HE staining in each group of KOA model rabbits were compared. Results: Compared with the model group, the aggregation of inflammatory cells in the moxibustion group and the inhibitor group was significantly reduced, but there was no significant difference between the two groups. The Lequesne scores of moxibustion group and inhibitor group were significantly lower than those of model group after 1 week, 2 weeks and 3 weeks of treatment(P<0.01). There was no significant difference in Lequesne score between moxibustion group and inhibitor group(P>0.05). The Lequesne scores of moxibustion group and inhibitor group after 1 week,2 weeks and 3 weeks of treatment were significantly lower than those before treatment(P <0.01), and the Lequesne scores of model group only after 3 weeks of treatment were lower than those before treatment(P<0.01).Conclusion: Both moxibustion and p38 inhibitor SB203580 can reduce the inflammation accumulation of chondrocytes in the knee joint of KOA rabbits, reduce local swelling and pain of the knee joint, and improve their gait and joint range of motion, suggesting that p38 MAPK signaling pathway may participate in the process of moxibustion treatment of knee osteoarthritis.
