摘 要:目的调查海南地区学龄儿童肠道菌群的结构、丰度与多样性,对比分析矮身材儿童与健康儿童肠道菌群的差异性及特点。方法选择海南省妇女儿童医学中心儿科门诊常规体检的儿童28例,其中14例为矮身材儿童组,14例为健康儿童组。采集受试者大便标本,提取基因组DNA,用特异性引物PCR扩增细菌16SrRNA基因V4区并高通量测序,进行菌群Alpha多样性分析。结果海南地区健康组儿童肠道菌群丰度ACE指数(P=0.007,R=124<130)和Chao1(P=0.0023,R=118<130)以及shannon指数(P=0.00000080,R=93<130)均显著高于矮身材儿童组(均P<0.01),矮身材组儿童的Simpson指数(P=0.000010,R=252>221)、显著高于健康组(P<0.01)。矮身材组儿童和健康组儿童在门属水平上的肠道菌群构成差异有统计学意义(P<0.01),厚壁菌门丰度健康组显著低于矮身材儿童组(R=218,P=0.018),放线菌门丰度健康组显著高于矮身材儿童组(R=212,P=0.044,P<0.05);在属水平上,健康组中的八叠球菌属(Sarcina R=138,P=0.04)和劳特氏菌属(Blautia R=137,P=0.031)丰度显著高于矮身材儿童组(均P>0.05),矮身材儿童组的巨单胞菌属(Megamonas R=213,P=0.029,)丰度显著高于健康组(P<0.05).结论海南地区矮身材儿童与健康儿童的肠道菌群在门和属水平上的构成及丰度均有不同,肠道菌群的结构异常可能是导致儿童身高发育迟缓的重要原因之一,可为儿童身高发育的预测和治疗提供理论参考。Objective To examine the diversity of intestinal flora in children of short stature and to ascertain the potential value of intestinal bacteria in the development of stature.Methods Subjects were 28 children undergoing a routine physical examination in the pediatrics clinic of this Hospital.Fourteen of the children were of short stature,and 14 were healthy controls.Fecal samples were collected,genomic DNA of microbes was extracted,and high-throughput sequencing was used to amplify the V4 region of the 16 sRNA gene.Microbial diversity and species composition were analyzed.Results Alpha analysis indicated that the ACE index(P=0.007,R=124<130),Chao1(P=0.0023,R=118<130),and Shannon index(P=0.00000080,R=93<130)in the healthy control group were significantly higher than those in children of short stature,and the indices differed significantly between the groups(P<0.01).The Simpson index(P=0.000010,R=252>221)in children of short stature was markedly higher than that in the healthy control group(P<0.01).At the genus level,the composition of intestinal flora differed between the children of short stature and the healthy control group.Firmicutes spp.were markedly less abundant in the healthy control group than in the children of short stature(R=218,P=0.018).Actinobacteria spp.were markedly more abundant in the healthy control group than in the children of short stature(R=212,P=0.044).Sarcina(R=138,P=0.04)and Blautia(R=137,P=0.031)spp.were markedly more abundant in the healthy control group than in the healthy control group.Megamonas spp.(R=213,P=0.029)were markedly more abundant in children of short stature than the healthy control group(P=0.029).Conclusion The composition and abundance of gut flora differed in children of different heights.An abnormal composition of intestinal flora may be a key cause of delayed development of stature in children.This finding can serve as a reference for predicting development of stature and treating related disorders in children.
