摘 要:目的检测2型糖尿病(T2DM)患者血清微小RNA-21-5p(miR-21-5p)与肠道菌群失调及炎症状态的关系。方法选取2018年6月-2019年6月本院收治的T2DM患者65例作为研究对象,另选取同期在本院进行体检的健康者45例作为对照组。收集受试者的一般资料进行比较分析;采集受试者血清,采用实时荧光定量PCR(qRT-PCR)检测miR-21-5p水平,酶联免疫吸附法(ELISA)检测受试者血清高敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、白介素-18(IL-18)水平。采集受检者粪便标本,采用平板培养法检测肠道肠球菌、肠杆菌、乳杆菌、双歧杆菌水平及双歧杆菌与肠杆菌的比值(B/E);Pearson法分析miR-21-5p与肠杆菌、乳杆菌、双歧杆菌、B/E、hs-CRP、TNF-α、IL-6、IL-18的相关性。结果与对照组相比,T2DM组血清miR-21-5p(2.34±0.35)、肠杆菌(9.08±0.65)、hs-CRP(3.41±0.62)mg/L、TNF-α(53.85±15.15)μg/L、IL-6(53.41±14.36)ng/ml、IL-18(215.62±42.46)μg/l水平升高(P<0.05);乳杆菌(9.41±0.36)、双歧杆菌(9.02±0.66)、B/E(0.96±0.19)水平降低(P<0.05)。Pearson法分析显示,miR-21-5p与肠杆菌、hs-CRP、TNF-α、IL-6、IL-18呈正相关(P<0.05),与乳杆菌、双歧杆菌、B/E水平呈负相关(P<0.05)。结论T2DM患者血清miR-21-5p水平与肠道菌群失调及炎症水平异常有关,可作为潜在的诊断指标。Objectives To determine the relationship between serum microRNA-21-5 p(miR-21-5 p)and an intestinal flora imbalance and inflammatory status in patients with type 2 diabetes mellitus(T2 DM).Methods Subjects were 65 patients with T2DM who were seen at this Hospital from June 2018 to June 2019.Forty-five healthy people who underwent a physical examination at this Hospital during the same period served as the control group.General data on subjects and controls were collected.Serum was collected.Real-time fluorescence quantitative PCR(qRT-PCR)was used to determine the serum level of miR-21-5 p and levels of Enterococcus,Enterobacter,Lactobacillus,and Bifidobacterium.The ratio of Bifidobacterium to Enterobacteriaceae(B/E)was determined using the plate culture method.Serum levels of highsensitivity C-reactive protein(hs-CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),and interleukin-18(IL-18)were determined using an enzyme-linked immunosorbent assay(ELISA).Pearson’s method was used to analyze the correlation between miR-21-5 p and Enterobacter,Lactobacillus,Bifidobacterium,B/E,hs-CRP,TNF-α,IL-6,and IL-18.Results Serum levels of miR-21-5 p(2.34±0.35),Enterobacter(9.08±0.65),hs-CRP(3.41±0.62 mg/L),TNF-α(53.85±15.15μg/L),IL-6(53.41±14.36 ng/ml),and IL-18(215.62±42.46μg/l)in the T2 DM group were significantly higher than those in the control group(P<0.05)while the levels of Lactobacillus(9.41±0.36)and Bifidobacterium(9.02±0.66)and the B/E(0.96±0.19)were lower(P<0.05).Pearson analysis indicated that miR-21-5 p was positively correlated with Enterobacter,hs-CRP,TNF-α,IL-6,and IL-18(P<0.05)but negatively correlated with levels of Lactobacillus and Bifidobacterium and the B/E(P<0.05).Conclusion The serum level of miR-21-5 p in patients with T2 DM is related to an intestinal flora imbalance and an abnormal level of inflammation.Serum miR-21-5 p may be used as apotential diagnostic index.
