基于《血证论》治血四法探讨过敏性紫癜的中医证治

摘　　要：过敏性紫癜的主要病机为热、瘀、虚,治疗以清热凉血、活血消瘀、养阴宁络、健脾补虚为治则,清热止血为第一要法,活血消瘀以防复发,养阴宁络以防血复动,健脾益气摄血以善后收功,且活血消瘀贯穿整个治疗过程。过敏性紫癜的中医证治充分验证了唐容川的止血四法:止血,消瘀,宁血,补虚,临证治疗具有显著疗效,故治血四法对过敏性紫癜的治疗具有重要的指导意义。辨证治疗过程中,治血四法的运用时机尤为重要。发病初期首先要辨明虚实,初期以邪实为主,根据血分热毒迫血妄行外溢的病因病机,总的治则是清热解毒、凉血止血、化瘀消斑,使热清血宁,瘀血得化,血行常道,则病得愈;病程日久多虚证或虚实夹杂,治宜益气健脾摄血、滋阴降火。The main pathogenesis of allergic purpura includes heat,blood stasis and deficiency.The principle of treatment is clearing heat and cooling blood,promoting blood circulation and removing blood stasis,nourishing yin and calming collaterals,strengthening spleen and tonifying deficiency.Promoting blood circulation and removing blood stasis is to prevent recurrence.Nourishing yin and calming collaterals is to prevent blood reactivation.Invigorating the spleen,replenishing qi and absorbing blood is to cope with the aftermath.Besides,promoting blood circulation and removing blood stasis carries out the whole treatment process.TCM syndrome differentiation and treatment of allergic purpura fully verifies Tang Rong-chuan′s four methods of treating blood diseases:hemostasis,removing blood stasis,calming blood,tonifying deficiency.The clinical treatment has significant curative effect,so the four methods of treating blood have important guiding significance for the treatment of Henoch Schonlein purpura:arresting bleeding,removing blood stasis,tranquilizing bleeding,and tonifying deficiency.Clinical treatment has significant curative effect,so the four methods of treating blood have important guiding significance for the treatment of allergic purpura.In the process of syndrome differentiation,the timing of the four methods is particularly important.In the early stage of the disease,the deficiency and excess should be identified first.In this stage,the main causes of the disease include pathogenic excess.According to the etiology and pathogenesis of blood aspect heat and toxin forcing blood to overflow,the general treatment is to clear away heat and toxin,cool blood and arrest bleeding,remove blood stasis and eliminate spots in order to clear heat and tranquilize bleeding,remove blood stasis,make the blood flow normal,then the disease is cured.If the course of the disease is long,and there is intermingled deficiency and excess,the treatment should focus on replenishing qi and invigorating spleen,absorbing blood,nourishing y
