归脾汤加减联合盐酸帕罗西汀片治疗心脾两虚型卒中后抑郁临床观察

摘　　要：目的观察归脾汤加减联合盐酸帕罗西汀片治疗心脾两虚型卒中后抑郁(PSD)的临床疗效。方法将104例心脾两虚型PSD患者按照随机数字表法分为2组,对照组52例予盐酸帕罗西汀片治疗,治疗组52例在对照组基础上联合归脾汤加减治疗。2组均治疗8周,比较2组治疗前后美国国立卫生研究院卒中量表(NIHSS)评分、日常生活能力量表(ADL)评分、匹兹堡睡眠质量指数量表(PSQI)评分、汉密尔顿抑郁量表(HAMD)评分及抑郁自评量表(SDS)评分变化情况,并比较2组治疗期间药物不良反应发生情况。结果与本组治疗前比较,2组治疗后NIHSS评分、PSQI评分、HAMD评分及SDS评分均明显降低(P<0.05),ADL评分均明显升高(P<0.05),且治疗组治疗后各项评分改善均优于对照组(P<0.05)。治疗组不良反应发生率1.92%(1/52),对照组不良反发生率9.62%(5/52),治疗组不良反应发生率低于对照组(P<0.05)。结论归脾汤加减联合帕罗西汀治疗心脾两虚型PSD疗效确切,可明显改善神经功能缺损症状,促进患者肢体功能恢复,提高日常生活能力,改善睡眠情况,改善抑郁状态,降低不良反应发生。Objective To observe clinical efficacy of Guipi Decoction combined with Paroxetine Hydrochloride Tablets in treating post-stroke depression complicated with deficiency of heart and spleen. Methods One hundred and four patients with post-stroke depression( PSD) complicated with dual heart-spleen deficiency syndrome were divided into two groups by means of the random number table method,52 patients in the control were treated with Paroxetine Hydrochloride Tablets,and 52 patients in the treatment group were subject to modified therapy combined with Guipi Decoction on the basis of the therapy applied for patients in the control group,the patients in both groups were treated for 8 weeks. The changes in national institutes of health stroke scale( NIHSS) scores,activity of daily living( ADL) scale scores,Hamilton depression scale( HAMD) scores,the depression self-rating scale( SDS) scores and Pittsburgh sleep quality index scale( PSQI) scores of patients before and after treatment were compared,and adverse drug reactions between the two groups were compared. Results NIHSS scores,PSQI scores,HAMD scores and SDS scores of patients in both two groups after treatment were significantly lowered when compared to such indexes of patients in respective group( P < 0. 05),ADL scores of patients in two groups significantly elevated when compared to such index of patients in respective group that before treatment( P < 0. 05),and the improvements of scores of patients in the treatment group were higher than such indexes of patients control group( P < 0. 05).The incidence of adverse reactions of patients in the treatment group and control group was was 1. 92%( 1/52)and 9. 62%( 5/52) respectively,and the incidence of adverse reactions of patients in the treatment group was lower than such index of patients in the control group. Conclusion Guipi Decoction combined with Paroxetine Hydrochloride Tablets delivers definite therapeutic effect in treating PSD complicated with dual heart-spleen deficiency,improve the neurological impairment s
