益气活血通脉汤联合单硝酸异山梨酯、阿司匹林治疗陈旧性心肌梗死临床评价

摘　　要：目的探讨益气活血通脉汤联合单硝酸异山梨酯、阿司匹林治疗陈旧性心肌梗死的临床疗效,以及对患者N末端脑利钠肽原(NT-proBNP)、肌酸激酶-同工酶(CK-MB)、乳酸脱氢酶(LDH)水平的影响。方法选取医院2018年1月至2020年1月收治的陈旧性心肌梗死患者84例,按随机数字表法分为观察组和对照组,各42例。两组患者均予单硝酸异山梨酯、阿司匹林治疗,观察组加用益气活血通脉汤治疗,两组患者均治疗6个月。结果观察组的总有效率为97.62%,显著高于对照组的80.95%(P<0.05);与对照组比较,治疗后观察组患者的超声心动图指标[心排血量(CO)、左心室射血分数(LVEF)、心排血指数(CI)]均显著更高,平均肺动脉压显著更低,纽约心脏病协会(NYHA)心功能分级改善更显著(P<0.05),NT-proBNP,CK-MB,LDH水平均显著更低(P<0.05);观察组和对照组不良反应发生率分别为11.90%和16.67%,差异无统计学意义(P>0.05)。结论益气活血通脉汤联合单硝酸异山梨酯、阿司匹林治疗陈旧性心肌梗死疗效显著,可明显改善患者超声心动图指标与心功能分级,降低NT-proBNP,CK-MB,LDH水平,且安全性较高。Objective To investigate the clinical efficacy of Yiqi Huoxue Tongmai Detection combined with isosorbide mononitrate and aspirin in the treatment of patients with old myocardial infarction(OMI)and its effect on the levels of N-terminal pro-brain natriuretic peptide(NT-proBNP),creatine phosphatase isoenzyme(CK-MB)and lactate dehydrogenase(LDH).Methods Totally 84 patients with OMI admitted to our hospital from January 2018 to January 2020 were selected and divided into the observation group and the control group according to the random number table method,42 cases in each group.The patients in the two groups were treated with isosorbide mononitrate and aspirin,on this basis,the patients in the observation group were treated with Yiqi Huoxue Tongmai Detection.Both groups were treated for 6 months.Results The total effective rate of the observation group was 97.62%,which was significantly higher than 80.95%of the control group(P<0.05).After treatment,the echocardiographic indexes[cardiac output(CO),left ventricular ejection fraction(LVEF)and cardiac output index(CI)]in the observation group were significantly higher than those in the control group,and the mean pulmonary artery pressure in the observation group was significantly lower than that in the control group.The New York Heart Association(NYHA)functional classification of the observation group was improved more significantly than that of the control group(P<0.05).The levels of NT-proBNP,CK-MB and LDH in the observation group were significantly lower than those in the control group(P<0.05).The incidence of adverse reactions in the observation group was similar to that in the control group(11.90%vs.16.67%,P>0.05).Conclusion Yiqi Huoxue Tongmai Detection combined with isosorbide mononitrate and aspirin is effective and safe in the treatment of patients with OMI,which can significantly improve the echocardiographic indexes and cardiac function classification,reduce the levels of NT-proBNP,CK-MB and LDH.
