摘　　要：目的探讨人巨细胞病毒(HCMV)感染新生儿后外周血和脑脊液中肿瘤坏死因子-α(TNF-α)、脂肪细胞补体相关蛋白30(Acrp30)、基质金属蛋白酶-3(MMP-3)水平及临床意义。方法选取2017年10月至2019年10月收治的196例HCMV感染新生儿(病例组),选取同期80例健康新生儿(对照组)。比较两组新生儿外周血和脑脊液中的TNF-α、Acrp30、MMP-3水平以及病例组治疗前、后各指标水平的变化,评估各指标对HCMV感染的预测价值。结果病例组新生儿外周血和脑脊液TNF-α、Acrp30、MMP-3水平均高于对照组,差异均具有统计学意义(P<0.05)。外周血和脑脊液TNF-α、Acrp30、MMP-3预测HCMV感染的灵敏度分别为75.51%、88.27%、83.16%、75.51%、82.14%、64.29%,特异度分别为81.15%、88.75%、80.00%、70.00%、87.50%、76.25%。治疗后,HCMV感染新生儿的外周血和脑脊液TNF-α、Acrp30、MMP-3水平均低于治疗前,差异均具有统计学意义(P<0.05)。结论外周血和脑脊液TNF-α、Acrp30、MMP-3水平可为新生儿HCMV感染以及预后评估提供参考,具有一定的临床辅助诊断价值。Objective To investigate the levels of tumor necrosis factor-α(TNF-α),adipocyte complement-related protein of 30(Acrp30)and matrix metalloproteinase-3(MMP-3)in peripheral blood and cerebrospinal fluid of neonates infected with human cytomegalovirus(HCMV)and their clinical significance.Methods Between October 2017 and October 2019,196 HCMV-infected neonates(case group)and 80 healthy neonates(control group)were enrolled in this study.The levels of TNF-α,Acrp30,and MMP-3 in peripheral blood and cerebrospinal fluid of the two groups were compared.And the levels of TNF-α,Acrp30,and MMP-3 of the case group before and after treatment were compared.The predictive value of each indicator for HCMV infection was evaluated.Results The levels of TNF-α,Acrp30,and MMP-3 in peripheral blood and cerebrospinal fluid in the case group were higher than those in the control group(P<0.05).The sensitivities of peripheral blood and cerebrospinal fluid TNF-α,Acrp30,and MMP-3 for predicting HCMV infection were 75.51%,88.27%,83.16%,75.51%,82.14%,and 64.29%,while the specificities were 81.15%,88.75%,80.00%,70.00%,87.50%,and 76.25%.After treatment,the levels of TNF-α,Acrp30 and MMP-3 in peripheral blood and cerebrospinal fluid in the case group were lower than those before treatment(P<0.05).Conclusion The levels of TNF-α,Acrp30 and MMP-3 in peripheral blood and cerebrospinal fluid can provide references for neonatal HCMV infection and prognosis assessment and have certain clinical auxiliary diagnostic value.
