系统性红斑狼疮合并自身免疫性溶血性贫血患者血常规中的红细胞聚集分析

摘　　要：目的分析系统性红斑狼疮合并自身免疫性溶血性贫血患者血常规中的红细胞聚集现象。方法选取本院2019年4月收治的1例系统性红斑狼疮合并自身免疫性溶血性贫血患者作为研究对象,选择全自动血细胞分析仪检测患者的血常规标本。将红细胞聚集标本进行37℃水浴,在水浴后不同时段重新上机进行检测。采用预稀释模式测定水浴前全血,并开展图片处理和瑞氏染色处理,对红细胞多项参数变化情况进行观察,对红细胞解聚情况进行镜下观察。结果与正常参考值相比,红细胞计数在全血水浴前后不同时段均明显降低,平均血红蛋白浓度、平均血红蛋白含量、平均红细胞体积均明显增加,镜下观察血涂片发现,红细胞均出现聚集现象;红细胞计数经预稀释处后明显上升,平均血红蛋白含量、平均红细胞体积基本正常,平均血红蛋白浓度恢复正常,对血涂片进行观察发现,红细胞基本表现为单独排列,散在分布。结论系统性红斑狼疮合并自身免疫性溶血性贫血患者全血在经长时间的37℃水浴后,依然无法解聚红细胞,利用预稀释处理则能获得较为满意的检测结果。Objective To analyze the red blood cell aggregation in the blood routine of patients with systemic lupus erythematosus and autoimmune hemolytic anemia.Methods one patient with systemic lupus erythematosus and autoimmune hemolytic anemia who was admitted to our hospital in April 2019 was selected as the research subjects,and the automatic blood cell analyzer was selected to detect the patient's routine blood samples.Red blood cell aggregation samples were placed in 37℃water bath,and then tested again at different time after the water bath.The whole blood before water bath was measured by pre dilution mode.At the same time,picture processing and Wright's staining were carried out to observe the changes of many parameters of red blood cells and the depolymerization of red blood cells under microscope.Results Compared with the normal reference value,the red blood cell count decreased significantly before and after the whole blood water bath,and the average hemoglobin concentration,the average hemoglobin content and the average red blood cell volume increased significantly.Under the microscope,the observation of the blood smear showed that the red blood cells had aggregation phenomenon;the red blood cell count increased significantly after the pre dilution,the average hemoglobin content and the average volume of red blood cells is basically normal,and the average hemoglobin concentration returns to normal.The observation of blood smear shows that the red blood cells are basically arranged separately and scattered.Conclusion The whole blood of patients with systemic lupus erythematosus and autoimmune hemolytic anemia can't depolymerize red blood cells after a long time of water bath in 37℃water bath,and the satisfactory results can be obtained by pre dilution treatment.
