流行性乙型脑炎临床特点及其早期干预措施对预后的影响

摘　　要：目的探讨流行性乙型脑炎患者超早期严格控制体温、及早有创呼吸机支持治疗、及早意识障碍康复训练的临床疗效,总结相关临床经验。方法选取该院2015年7月至2017年10月收治的流行性乙型脑炎患者共56例,根据治疗方法不同分为对照组(n=32)和治疗组(n=24)。对照组给予常规治疗,必要时行气管插管并予有创呼吸机治疗;治疗组予以超严格控制体温、及早进行气管插管并予有创呼吸机治疗、及早开展意识障碍康复训练等措施,对2组患者的疗效、预后进行对比。结果治疗组患者体温、呼吸频率、氧分压、脉搏血氧饱和度、格拉斯哥评分均高于对照组,差异均有统计学意义(P<0.05)。结论在基础治疗的同时施行超早期严格控制体温、早期有创呼吸机支持治疗、及早意识障碍康复训练可获得显著疗效。Objective To explore the clinical efficacy of ultra-early strict control of body temperature,early invasive ventilator support therapy,and early consciousness disorder rehabilitation training in patients with epidemic encephalitis B,and to summarize relevant clinical experience.Methods A total of 56 cases of epidemic encephalitis B patients of the department from July 2015 to October 2017 were selected and divided into control group(n=32)and treatment group(n=24)according to different treatment methods.The control group received routine treatment,if necessary,tracheal intubation and invasive ventilator treatment.The treatment group ultra-early strictly controlled body temperature,early tracheal intubation and invasive ventilator treatment,and early rehabilitation of consciousness disorder,etc.The efficacy and prognosis of the two groups of patients were compared.Results The body temperature,respiratory rate,oxygen partial pressure,pulse oxygen saturation Glasgow score were higher in the treatment group than those in the control group,the differences were statistically significant(P<0.05).Conclusion At the same time of basic treatment,strict control of body temperature,early invasive ventilator support therapy,and early consciousness disorder rehabilitation training can achieve significant curative effect.
