老年高血压患者脑血流动力学参数、血液指标变化及脑白质疏松发生情况研究

摘　　要：目的研究老年高血压患者脑血流动力学参数、血液指标变化及脑白质疏松发生情况。方法选取濮阳市人民医院收治的64例老年高血压患者,根据高血压严重程度分为轻中度组(n=32)和重度组(n=32),并选取同期正常体检的32例老年人群为对照组,观察三组患者的脑血流动力学参数[大脑动脉平均血流速度(Vm)、舒张期末血流速度(Vd)、收缩期峰值血流速度(Vs)、搏动指数(PI)]、血液指标[全血高切黏度、全血低切黏度、血浆黏度、红细胞压积]及脑白质疏松发生率。结果重度组、轻中度组的脑血流动力学参数Vm、Vd、Vs、PI均显著高于对照组,且重度组Vm、Vd、Vs、PI显著高于轻中度组,差异均显著(P<0.05)。重度组、轻中度组的血液指标全血高切黏度、全血低切黏度、血浆黏度及红细胞压积均显著高于对照组,且重度组全血高切黏度、全血低切黏度、血浆黏度及红细胞压积显著高于轻中度组,差异均显著(P<0.05)。重度组、轻中度组的脑蛋白疏松发生率均显著高于对照组,且重度组脑蛋白疏松发生率显著高于轻中度组,差异均显著(P<0.05)。结论老年高血压患者脑血流动力学参数、血液指标及脑白质疏松发生率均与病情关系密切,可帮助临床判断高血压病情。Objective To study the changes of cerebral hemodynamics parameters,blood parameters and leukoaraiosis in elderly patients with hypertension.Methods 64 elderly patients with hypertension admitted to Puyang People's Hospital were divided into mild-to-moderate group(N=32)and severe group(N=32)according to the severity of hypertension,the cerebral hemodynamics parameters[mean cerebral artery blood flow velocity(Vm),end diastolic blood flow velocity(Vd),peak systolic blood flow velocity(Vs)and pulsatility index(PI)],blood parameters(whole blood high shear viscosity,whole blood low shear viscosity,plasma viscosity,hematocrit)and incidence of leukoaraiosis were observed.Results The cerebral hemodynamics parameters Vm,Vd,Vs,PI of the severe group and the light and moderate group were significantly higher than those of the control group(P<0.05).The whole blood high shear viscosity,whole blood low shear viscosity,plasma viscosity and hematocrit of the severe group and the light and moderate group were significantly higher than those of the control group,the whole blood high shear viscosity,whole blood low shear viscosity,plasma viscosity and hematocrit in severe group were significantly higher than those in mild and moderate group(P<0.05).The incidence of cerebral protein loose in severe group and mild-moderate group was significantly higher than that in control group,and the incidence of cerebral protein loose in severe group was significantly higher than that in mild-moderate group(P<0.05).Conclusion Brain hemodynamics parameters,blood parameters and incidence of leukoaraiosis are closely related to the condition of hypertension in the elderly,which can help to judge the condition of hypertension.
