摘 要:目的急诊危重病人纾解困难已成为现代急诊医学发展的世界性难题,在我国大型公立医院尤其突出。分析该现象的现状以及原因,探讨解决问题的路径,对于改善我国急诊医疗环境、缓解三级公立医院的医患矛盾有着现实意义。方法从国内外急诊滞留现状展开,分析成因、厘清功能定位,提供优化建议。结果医患双方对"急诊"认识不足,急诊收治流程有待规范,急诊"硬件"资源的不足,急诊"软件"资源的欠缺,科室床位周转率低下,分级诊疗制度未落实到位。结论提升病人的流入效率,提高病人的急救效果,加快病人的流出进程。Objective Emergency relief of critically ill patients has become a worldwide problem in the development of modern emergency medicine,especially in large public hospitals in China.It is of practical significance to analyze the current situation and causes of this phenomenon,and to explore the ways to solve the problem,so as to improve the emergency medical environment in China and alleviate the doctor-patient conflict in tertiary hospitals.Methods Based on the current situation of emergency detention at home and abroad,analyzing the causes and clarifying the functional positioning,so as to provide optimization suggestions.Results Both doctors and patients have insufficient understanding of"emergency",the emergency treatment process needs to be standardized,the emergency"hardware"resources are insufficient,the emergency"software"resources are insufficient,the turnover rate of beds in departments is low,and the hierarchical diagnosis and treatment system is not in place.Conclusion It is suggested to improve the inflow efficiency of patients,improve the first aid effect of patients,and speed up the outflow process of patients.
