椎体成形术结合补肾壮骨汤治疗高龄骨质疏松性胸腰椎压缩骨折的临床观察

摘　　要：目的:探讨分析椎体成形术结合补肾壮骨汤治疗高龄骨质疏松性胸腰椎压缩骨折的临床疗效。方法:选取2017年2月-2019年10月收治的76例骨质疏松性胸腰椎压缩性骨折高龄患者随机分为观察组和对照组,观察组给予椎体成形术结合补肾壮骨汤治疗,对照组仅给予椎体成形术。比较两组临床疗效,治疗前后影像学指标、骨密度、功能障碍指数及疼痛评分。结果:观察组临床总有效率较对照组显著提高,差异具有统计学意义(P <0.05);治疗前两组患者椎体前缘高度丢失百分比、椎体中部高度丢失百分比差异无统计学意义(P> 0.05),治疗后两组椎体前缘高度丢失百分比、椎体中部高度丢失百分比均显著降低,且观察组降低更显著,差异均具有统计学意义(P <0.05);治疗前两组骨密度、Oswestry功能障碍指数(Oswestry Disability Index,ODI)及疼痛视觉模拟评分法(Visual Analogue Scale,VAS)评分差异无统计学意义(P> 0.05),治疗后两组骨密度、Oswestry指数及VAS评分均显著改善,且观察组骨密度、Oswestry指数及VAS评分改善更显著,差异均具有统计学意义(P <0.05)。结论:椎体成形术结合补肾壮骨汤治疗高龄骨质疏松性胸腰椎压缩骨折可显著提高临床疗效,纠正脊柱后凸畸形,缓解患者疼痛,促使骨密度增加,改善胸腰椎功能,值得在临床上推广。Objective: To explore and analyze the clinical effect of vertebroplasty plus the Bushen Zhuanggu decoction( 补肾壮骨汤) on elderly osteoporotic thoracolumbar compression fractures. Methods: 76 cases admitted from February 2017 to October 2019 were randomly divided into the observation group and the control group. The observation group was treated by vertebroplasty and the Bushen Zhuanggu decoction. The control group received only vertebroplasty. Results: The total efficiency in the observation group was significantly higher than that in the control group(P<0.05). The anterior vertebral height loss percentage and mid-vertebral height loss percentage between the groups in the groups were significantly reduced, and the reduction in the observation group was more significant(P<0.05). Bone density, Oswestry disability index scores and VAS scores in the groups were all significantly improved, and in the observation group were improved more significantly, P<0.05. Conclusion: Vertebroplasty plus the Bushen Zhuanggu decoction on elderly osteoporotic thoracolumbar compression fractures can significantly improve the clinical efficacy, correct kyphosis, relieve patient pain, increase bone density, improve thoracolumbar spine function, and is worthy of clinical promotion.
