摘　　要：目的应用声辐射力脉冲(acoustic radiation force impulse,ARFI)成像技术评估兔脑死亡(brain death,BD)状态下肝脏剪切波速度(shear wave velocity,SWV),探讨评估BD供肝的新方法。方法制备兔BD组和假手术组模型并维持状态达8 h,应用ACUSON S2000超声仪,测量BD组和假手术组模型制备完成后0 h、4 h、8 h的肝脏SWV值,同时进行肝功能和病理检查,并进行统计分析。结果BD组和假手术组模型制备完成后0 h、4 h、8 h肝脏SWV值分别为(1.22±0.28)m/s、(1.37±0.23)m/s、(1.69±0.47)m/s和(1.09±0.19)m/s、(1.25±0.11)m/s、(1.32±0.19)m/s。BD后肝脏SWV随时间推移逐渐升高,BD组与假手术组术后8h的SWV相比较差异有统计学意义(P<0.05)。结论兔BD状态下肝脏质地逐渐变硬,SWV可作为辅助评估BD供肝的新技术。Objective To evaluate the shear wave velocity(SWV)of the rabbits liver in the condition of brain death(BD)by acoustic radiation force impulse(ARFI)imaging and to explore a new method for assessing BD donor liver.Methods The models of BD group and sham operation group were prepared and to be maintained for 8 hours.We measured liver SWV values of BD group and sham operation group by ACUSON S2000 at 0 h,4 h and 8 h after the model preparation completed.Liver function and pathology were performed simultaneously.Then,the data were analyzed.Results Liver SWV values of BD group and sham operation group at 0 h,4 h and 8 h were(1.22±0.28)m/s,(1.37±0.23)m/s,(1.69±0.47)m/s and(1.09±0.19)m/s,(1.25±0.11)m/s,(1.32±0.19)m/s,respectively.After BD,liver SWV increased gradually with time.There was a statistically significant difference between BD group and sham group in SWV at 8 h after surgery(P<0.05).Conclusion Rabbit liver texture gradually hardens under BD state,and SWV can be used as a new auxiliary technique to evaluate BD donor liver.
