地黄汤加减对紫癜性肾炎患儿疗效、炎性因子及免疫功能的影响

摘　　要：目的:探讨地黄汤加减对紫癜性肾炎患儿疗效、炎性因子及免疫功能的影响。方法:选择2018年1月~2020年1月在本院接受治疗的紫癜性肾炎患儿98例为研究对象。采用随机数字表法将患儿分为观察组与对照组,每组49例。对照组参照《紫癜性肾炎诊治循证指南(2016)》的相关诊断标准给予常规西医治疗,观察组在此基础上加以地黄汤加减治疗。比较临床疗效、炎性因子、免疫功能及不良反应。结果:观察组总有效率(85.71%)高于对照组(67.35%,P <0.05)。治疗后两组患儿白细胞介素-2(IL-2)水平均升高,观察组升高幅度更大;白细胞介素-4(IL-4)及肿瘤坏死因子-α(TNF-α)水平均下降,观察组下降更明显(P <0.05);CD4^+水平均升高,观察组升高幅度更大;CD8^+水平均下降,观察组下降幅度更大(P <0.05);对照组未见明显不良反应,观察组出现2例(4.08%)不良反应,两组无统计学差异(P> 0.05)。结论:地黄汤加减联合常规西医治疗较单独使用常规西医治疗,可有效提高紫癜性肾炎患儿疗效、改善患儿炎症水平并提高免疫功能,且不增加不良反应发生率。Objective:To investigate the effect of Dihuang decoction on the efficacy,inflammatory factors and immune function of children with purpura nephritis.Methods:From January 2018 to January 2020,98 children with purpuric nephritis treated in our hospital were included in the study.Random number table method was used to divide patients into 2 groups,49 cases in each group.The control group was given conventional chemical drugs treatment according to the relevant provisions of the Evidence-Based Guidelines for Diagnosis and Treatment of Purpuric Nephritis(2016),while the observation group was treated with additional Dihuang decoction on that basis.Clinical efficacy,inflammatory factors,immune function and adverse reactions were compared.Results:The total effective rate in the observation group was 85.71%,which was higher than 67.35%in the control group(P<0.05).After treatment,the interleukin-2(IL-2)level of the two groups of patients increased significantly,with more increase found in the observation group.The interleukin-4(IL-4)and tumor necrosis factor-α(TNF-α)levels of the two groups decreased significantly,with more reduction found in the observation group(P<0.05).After treatment,the CD4+level of the two groups of children increased significantly,with more increase obtained in the observation group(P<0.05).After treatment,the CD8+level of the two groups of children decreased significantly,with more reduction revealed in the observation group(P<0.05).There were no obvious adverse reactions in the control group,while 2 cases of adverse reaction(4.08%)were found in the observation group,with no significant difference(P>0.05).Conclusion:Dihuang decoction combined with conventional chemical drugs treatment can effectively improve the curative effect in children with purpuric nephritis,improve the inflammation level and improve immune function without increasing the incidence of adverse reactions.
