超声下前路腰、骶丛阻滞在老年全髋置换中的应用研究

摘　　要：目的探讨超声引导下前路腰、骶丛阻滞在老年全髋关节置换中的临床应用价值。方法选取2018年5月至2019年8月该院100例老年全髋关节置换患者为研究对象,分为观察组和对照组,各50例。对照组行椎管内穿刺,观察组实施超声引导下前路腰、骶丛阻滞。比较两组麻醉起效时间及感觉功能阻滞持续时间、术后感觉与运动功能恢复时间、麻醉术前准备时间、麻醉操作时间及术后住院时间及相关并发症,分析两组不同时间点疼痛数字评分变化情况。结果与对照组比较,观察组麻醉起效时间更快,感觉功能阻滞持续时间更长,感觉与运动功能恢复时间更早,麻醉前准备与操作时间更短,术后住院时间更短,麻醉中及麻醉术后并发症发生率更低,且两组不同时间点疼痛数字评分比较,差异有统计学意义(P<0.05)。结论超声下前路腰、骶丛阻滞有一定临床价值。Objective To explore the clinical value of ultrasound-guided anterior lumbar and sacral plexus block in elderly total hip replacement.Methods A total of 100 cases with total hip replacement performed from May 2018 to August 2019 were selected and divided into two groups with 50 cases each.The control group was received spinal canal puncture and continuous epidural anesthesia.The observation group was received ultrasound-guided anterior lumbar and sacral plexus block.Then compared the onset time of anesthesia and the duration of sensory function block,postoperative sensory and motor function recovery time,preparation time before anesthesia,anesthesia operation time,postoperative hospital stay and related complications.The pain at different time points in the two groups was analyzed.Results Compared with the control group,the observation group has faster onset time of anesthesia,longer duration of sensory function block,earlier recovery time of sensory and motor functions,shorter preparation and operation time before anesthesia,shorter postoperative hospital stay,and lower the incidence of complications after anesthesia,there was statistically significant difference between the two groups of pain numerical scores at different time points(P<0.05).Conclusion Anterior lumbar and sacral plexus block under ultrasound has certain clinical value.
