国医大师刘志明辨治新冠肺炎危重患者典型病案研析

摘　　要：国医大师刘志明从医八十余年,对于中医防治传染病有着深入的研究和丰富的实践经验,以“表里双解治温病”“重症关键在于祛邪”等为特色。刘教授心系新型冠状病毒肺炎(corona virus disease 2019,COVID-19)疫情防控工作,远程会诊一线COVID-19危重患者。选方常用苇茎汤(《备急千金药方》)、杏苏散、白虎汤、生脉散、清营汤、猪苓汤、三消饮等经典名方,前、中期痰热严重选加黄芩半夏丸、贝母瓜蒌散、茯苓杏仁甘草汤的药物,恢复期加黄芪、西洋参等养护气阴。经治患者已经全部治愈出院,出院后随访无1例复发或核酸检测复阳。通过分析典型病案,总结提炼刘教授对温病的辨证论治思路,供医务工作者参考。Traditional Chinese medicine master Liu Zhiming has been practicing medicine for 80 years,he has in-depth research and rich practical experience in the prevention and treatment of infectious diseases by traditional Chinese medicine methods,which is characterized by"treating febrile diseases with resolve both the exterior and interior"and"eliminating pathogenic factors is the key to severe diseases".Professor Liu was committed to the prevention and control of corona virus disease 2019(COVID-19)epidemic and conducted remote consultation of critical COVID-19 patients on the front line.The commonly used prescriptions are Qianjin Weijing Decoction(Essential Recipes for Emergent Use Worth a Thousand Gold),Xingsu Powder,Baihu Decoction,Shengmai Powder,Qingying Decoction,Zhuling Decoction,Sanxiao Decoction,etc.In the early and middle stage,the patients with severe phlegm heat were treated with Huangqin Banxia Pill,Beimu Gualou Powder,Fuling Xingren Gancao Decoction.In the recovery period,Huangqi(Astragali Radix)and Xiyangshen(Panacis Quinquefolii Radix)were added to maintain qi and Yin.After careful treatment,all patients had been discharged from the hospital,and none of them were recurrence or nucleic acid test positive.By analyzing typical cases,we summarized and refined professor Liu's thoughts of syndrome differentiation and treatment of febrile diseases for the reference of medical workers.
