超声引导下针刺治疗慢性非特异性下腰痛的临床疗效观察

摘　　要：目的观察超声引导针刺治疗慢性非特异性下腰痛(chronic non-specific low back pain,CNSLBP)的临床疗效。方法将60例CNSLBP患者随机分为治疗组(超声引导针刺组)、对照组(普通针刺组),各30例,经治疗后观察两组的视觉模拟评分(visual analogue scale,VAS)值、日本骨科协会(Japanese Orthopaedic Association,JOA)评分值、杨氏模量值及竖脊肌肌筋膜厚度值的变化,并对比两种方法治疗CNSLBP的临床疗效。结果治疗后,两组患者VAS评分、杨氏模量值均较治疗前降低,JOA评分较治疗前升高,差异均有统计学意义(P<0.05);且治疗后治疗组的VAS评分、杨氏模量值低于对照组,JOA评分高于对照组,差异均有统计学意义(P<0.05);两组患者治疗前后及组间竖脊肌肌筋膜厚度值比较,差异均无统计学意义(P>0.05);治疗组总有效率高于对照组,差异有统计学意义(P<0.05)。结论超声引导下针刺治疗CNSLBP疗效显著,运用肌骨超声诊断和治疗CNSLBP患者值得临床推广。Objective To observe the clinical effect of ultrasound-guided acupuncture on chronic non-specific low back pain(CNSLBP).Methods 60 patients with CNSLBP were randomly divided into the treatment group(ultrasound-guided acupuncture group)and the control group(ordinary acupuncture group),with 30 cases in each group.After treatment,the visual analogue scale(VAS)value,Japanese Orthopaedic Association(JOA)score,Young's modulus value and erector spinae myofascial thickness of the two groups were observed,and the clinical efficacy of the two methods in the treatment of CNSLBP was compared.Results After treatment,VAS score and Young's modulus value of the two groups were lower than before treatment,JOA score was higher than before treatment,the differences were statistically significant(P<0.05);and after treatment,VAS score and Young's modulus value of the treatment group were lower than those of the control group,JOA score was higher than those of the control group,the differences were statistically significant(P<0.05);the thickness of erector spinae muscle fascia of the two groups before and after treatment and between groups,the differences were not statistically significant(P>0.05);the total effective rate of the treatment group was higher than that of the control group,the difference was statistically significant(P<0.05).Conclusion Ultrasound-guided acupuncture has a significant effect on CNSLBP,and the application of musculoskeletal ultrasound in the diagnosis and treatment of CNSLBP is worthy of clinical promotion.
