摘  要:[目的]为引导社会办医、规范医院市场竞争提供参考。[方法]收集2014-2018年我国31个省份人均医疗费用和病死率数据,从费用和质量2个维度反映医院市场的竞争效果,通过固定效应模型和随机效应模型研究民营医院参与我国医疗服务市场竞争后的实际竞争效果。[结果]市场集中度对门诊费用的影响系数为-0.344且显著,对住院费用的影响系数不显著;市场集中度对观察室病死率和住院病死率的影响均显著为正,系数分别为3.098和0.790。[结论]民营医院的发展加剧了我国医疗服务市场的竞争程度;医院市场竞争程度的增加有利于提升医疗质量,但同时也造成医疗费用的提高;我国医疗服务市场的竞争形式主要集中在质量竞争。Objective To provide a reference for social capital hospital and standard hospital market competition. Methods It collected per capita medical expenses and fatality rate data in 31 provinces from 2014 to 2018. It studied actual competition effect after private hospital participating in the competition of medical service market through fixed effects model and random effects model. Results The influence coefficient of market concentration on outpatient expenses was-0.344, which was significance, but the influence coefficient on hospitalization expense was not significance. The influence of market concentration on observation room and inpatient fatality rate was both significantly positive, with coefficients of 3.098 and 0.790 respectively. Conclusions The development of private hospital has intensified the competition in China’s medical service market. The increase of market competition in favour of the promotion of medical quality, but it causes the increase of medical expense. The competitive form of China’s medical service market is mainly concentrated on the competition of quality.
