摘 要:目的探讨吲哚美辛对大鼠跟腱创伤后异位骨化(HO)的影响及机制。方法将30只雄性SPF级SD大鼠随机分为药物组、模型组、对照组各10只。药物组和模型组行左后肢跟腱切断诱导HO模型,对照组仅切开皮肤未切断跟腱。造模第1天,药物组给予吲哚美辛10 mg(/kg·d)强制灌胃,模型组与对照组给予同等体积生理盐水强制灌胃,均1次/d,连续干预4周。造模前及造模第5、10周,各组行X线片检查,观察大鼠左跟腱HO情况并对HO进行分级;分别于造模前及造模第5、10周清晨抽取各组麻醉下左侧股静脉血,用酶联免疫吸附法检测血清SO。造模第5、10周每组处死5只大鼠取左侧跟腱行大体形态观察。结果造模第5周模型组7只、药物组2只出现骨化影,对照组未见骨化影;造模第10周模型组5只、药物组2只出现骨化影,对照组未见骨化影。造模第5、10周,与对照组比较,模型组HO数量及骨化程度分级均高(P均<0.05);与模型组比较,药物组HO数量及骨化程度分级均低(P均<0.05)。造模第5、10周,与对照组比较,模型组血清SO水平低(P均<0.05);与模型组比较,药物组血清SO水平高(P均<0.05)。术后5、10周对照组跟腱呈白色,质地韧;模型组大鼠跟腱呈浅棕黄色,质地硬;药物组大鼠跟腱组织体积及质地介于对照组及模型组之间。结论吲哚美辛可抑制大鼠跟腱创伤后HO的发生,其作用机制可能与SO表达上调有关。Objective To investigate the effect and mechanism of indomethacin on heterotopic ossification(HO)after Achilles tendon trauma in rats.Methods Thirty male Sprague SPF SD(Sprague Dawley)rats were randomly divided into the experimental group,model group,and control group,with 10 in each.In aseptic conditions,Achilles tenotomy in the left hind leg was conducted among the rats in the experimental group and the model group to induce the HO models while rats in the control group just underwent skin incision instead of the Achilles tendon cutting.On the first day of modeling,rats in the experimental group took 10 mg(/kg·d)indomethacin concentrated solution by gavage,while rats in the model group and the control group were given an equal volume of normal saline,once a day,for 4 weeks.Before modeling and at the 5th and 10th weeks of modeling,X-ray examination was performed in each group to observe the HO of left Achilles tendon and grade HO.The blood samples of left femoral vein were collected before modeling and in the morning of the 5th and 10th weeks after modeling.Serum SO was measured by ELISA.Five rats were sacrificed for gross morphological observation of their left achilles tendons in each group.Results The comparison of X-rays in the fifth week showed that,ossification was found in 7 rats in the model group and 2 rats in the experimental group,which was not observed in the control group.At the tenth week,ossification was found in 5 rats of the model group and 2 rats in the experimental group,which was not observed in the control group.Compared with the control group,the number of HO and the degree of ossification in the model group were higher than those in the control group(both P<0.05);compared with the model group,the number of HO and the degree of ossification in the drug group were lower(both P<0.05).At the 5th and 10th weeks,compared with the control group,the serum SO level of the model group was lower(P<0.05),and the serum SO level of the drug group was higher than that of the model group(P<0.05).At the 5
