升清降浊胶囊对慢性肾衰竭患者肾功能及生活质量的影响

摘　　要：目的观察升清降浊胶囊对慢性肾衰竭(CRF)患者肾功能和生活质量的影响。方法将92例CRF患者按照随机数字表法分为2组。对照组46例(后剔除7例)予常规治疗,治疗组46例(后剔除5例)在对照组治疗基础上予升清降浊胶囊治疗。2组疗程均为4周。比较2组治疗前后中医证候评分、血红蛋白(Hb)、血钾(K)、血钙(Ca)、血磷(P)、血肌酐(Cr)、血清尿素氮(BUN)、内生肌酐清除率(Ccr)、血清甲状旁腺激素(PTH)、健康调查简表(SF-36)及临床安全性指标[血压、总胆固醇(TC)、甘油三酯(TG)、丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)]。结果治疗后治疗组头晕、疲倦乏力、气短懒言、腰痠膝软、肢体困重、食少纳呆、口干、口苦、口中黏腻、夜尿、肢体麻木、面色晦黯、腰痛评分及总分均较本组治疗前降低(P<0.05),对照组头晕、疲倦乏力、气短懒言、肢体困重、食少纳呆、口苦、口中黏腻、水肿、夜尿、肢体麻木、面色晦黯、腰痛评分及总分均降低(P<0.05);治疗后治疗组疲倦乏力、腰痠膝软、肢体困重、夜尿评分及总分均低于对照组(P<0.05)。治疗后2组Hb、血Ca、Ccr均较本组治疗前升高(P<0.05),治疗组血K、血P、PTH、Cr、BUN均降低(P<0.05),对照组血K、血P、PTH、Cr均降低(P<0.05);治疗后治疗组Hb、血Ca、Ccr均高于对照组(P<0.05),血P、PTH、Cr均低于对照组(P<0.05)。治疗后治疗组SF-36中各单项评分及总分均较本组治疗前升高(P<0.05),对照组仅情感职能评分升高(P<0.05);治疗后治疗组生理职能、精神健康、情感职能评分均高于对照组(P<0.05)。治疗后治疗组收缩压、TG较本组治疗前降低(P<0.05),治疗后治疗组TG低于对照组(P<0.05)。结论升清降浊胶囊可以改善CRF患者的临床症状及肾功能指标,提高生活质量,且具有较高的安全性。Objective To observe the effect of Shengqing Jiangzhuo Capsule on renal function and quality of life in patients with chronic renal failure(CRF).Methods 92 CRF patients were randomly divided into two groups.46 cases in the control group(7 cases were excluded)were given conventional treatment,and 46 cases in the treatment group(5 cases were excluded)were given Shengqing Jiangzhuo Capsule more.Both groups were treated for 4 weeks.The TCM syndrome score,blood potassium(K),calcium(Ca),phosphorus(P),serum creatinine(Cr),blood urea nitrogen(BUN),endogenous creatinine clearance rate(Ccr),Parathyroid hormone(PTH),short form of health survey(SF-36 scale)and clinical safety indexes[blood pressure,total cholesterol(TC),triglyceride(TG),alanine aminotransferase(ALT),aspartate aminotransferase(AST)]of the two groups were compared between the two groups before and after treatment.Results After treatment,the scores of dizziness,fatigue,shortness of breath,weakness of waist and knee,body sleepiness,poor appetite,dryness in the mouth,bitter taste,sticky and greasy in mouth,nocturia,numbness and weakness,dull and sallow complexion,low back pain score and total score were lower than those before treatment(P<0.05).In the control group,dizziness,fatigue,shortness of breath,body sleepiness,poor appetite,bitter taste,sticky and greasy in mouth,edema,nocturia,numbness and weakness,dull and sallow complexion,low back pain score and total score were decreased(P<0.05).After treatment,the scores of fatigue,weakness of waist and knee,body sleepiness,nocturia and total score of the treatment group were lower than those of the control group(P<0.05).After treatment,Hb,Ca and Ccr in the two groups were higher than those before treatment(P<0.05),K,P,PTH,Cr and BUN in the treatment group were lower than those before treatment(P<0.05),and K,P,PTH and Cr in the control group were lower than those before treatment(P<0.05).After treatment,Hb,Ca and CCR in the treatment group were higher than those in the control group(P<0.05),and P,PTH and Cr in the t
