丹栀逍遥散加减治疗消化性溃疡肝胃郁热证临床疗效及对患者血清炎症因子和胃肠激素的影响

摘　　要：目的观察丹栀逍遥散加减治疗消化性溃疡肝胃郁热证的临床疗效及对患者血清炎症因子和胃肠激素的影响。方法将104例消化性溃疡肝胃郁热证患者按照随机数字表法分为2组。对照组51例予西医四联疗法,治疗组53例在对照组治疗基础上联合丹栀逍遥散加减治疗。2组疗程均为2周。2组治疗前后比较血清炎症因子[白细胞介素6(IL-6)、超敏C反应蛋白(hs-CRP)、肿瘤坏死因子α(TNF-α)]、胃肠激素[胃泌素(GAS)、胃动素(MTL)、血管活性肽(VIP)]水平,并统计疗效及不良反应发生情况。结果治疗组总有效率94.34%(50/53),对照组总有效率80.39%(41/51),治疗组疗效优于对照组(P<0.05)。治疗后2组血清IL-6、hs-CRP、TNF-α水平均较本组治疗前降低(P<0.05),且治疗后治疗组均低于对照组(P<0.05)。治疗后2组GAS、MTL水平均较本组治疗前降低(P<0.05),VIP水平均升高(P<0.05),且治疗后治疗组GAS、MTL水平低于对照组(P<0.05),VIP水平高于对照组(P<0.05)。治疗组不良反应发生率5.66%(3/53),对照组不良反应发生率21.57%(11/51),治疗组不良反应发生率低于对照组(P<0.05)。结论丹栀逍遥散加减治疗消化性溃疡肝胃郁热证疗效确切,并能减少不良反应的发生,可能与抑制炎性反应、调节胃肠激素等因素有关。Objective To observe the clinical efficacy of Danzhi Xiaoyao Powder in the treatment of liver-stomach stagnation-heat syndrome of peptic ulcer and its influence on serum inflammatory factors and gut hormones of patients.Methods:A total of 104 patients with liver-stomach stagnation heat syndrome of peptic ulcer were randomly divided into two groups.the control group were treated with western medicine quadruple therapy(n=51),and the treatment group(n=53)were treated with Danzhi Xiaoyao Powder on the basis of the control group.Both groups were treated for 2 weeks.Before and after treatment,the levels of serum inflammatory factors[interleukin 6(IL-6),high-sensitivity C-reactive protein(hs-CRP),tumor necrosis factorα(TNF-α)],gastrointestinal hormones[gastrin(GAS),motilin(MTL),and vasoactive peptide(VIP)]were compared between the two groups,and the efficacy and adverse reactions were counted.Result The total effective rate of the treatment group was 94.34%(50/53),and that of the control group was 80.39%(41/51).The curative effect of the treatment group was better than that of the control group(P<0.05).After treatment,the levels of serum IL-6,hs-CRP and TNF-αin the two groups were lower than those before treatment(P<0.05),and those in the treatment group were lower than the control group(P<0.05).After treatment,the levels of GAS and MTL in the two groups were lower than those before treatment(P<0.05),and the level of VIP was increased(P<0.05).After treatment,the levels of GAS and MTL in the treatment group were lower than those in the control group(P<0.05),and the level of VIP was higher than that in the control group(P<0.05).The incidence of adverse reactions in the treatment group was 5.66%(3/53),and that in the control group was 21.57%(11/51).The incidence of adverse reactions in the treatment group was lower than that in the control group(P<0.05).Conclusion Modified Danzhi Xiaoyao Powder is effective in the treatment of peptic ulcer with stagnation of liver and stomach heat syndrome,and it can reduce the occurren
