儿制动汤加减联合生物反馈治疗儿童抽动障碍的临床研究

摘　　要：目的探讨儿制动汤加减联合生物反馈治疗儿童抽动障碍的临床疗效。方法将90例抽动障碍儿童按照随机数字表法分为2组。对照组45例予盐酸硫必利片治疗,治疗组45例予儿制动汤加减联合生物反馈治疗。2组均治疗12周。比较2组临床疗效、治疗前后中医证候评分、耶鲁综合抽动严重程度量表(YGTSS)评分、不良反应发生率、复发率。结果治疗组总有效率91.11%(41/45),对照组总有效率75.56%(34/45),治疗组疗效优于对照组(P<0.05)。2组治疗后发声抽动、运动抽动、纳差、烦躁易怒、精神怠倦等中医证候评分与本组治疗前比较均明显降低(P<0.05),且治疗组治疗后各项评分均低于对照组(P<0.05)。2组治疗后发声性抽动、运动性抽动、综合性损伤等评分及YGTSS总分与本组治疗前比较均明显降低(P<0.05),且治疗组治疗后各项评分均低于对照组(P<0.05)。治疗组不良反应发生率4.44%(2/45),对照组不良反应发生率20.00%(9/45),治疗组不良反应发生率低于对照组(P<0.05)。治疗组治疗后6个月、1年复发率分别为4.88%、7.32%,对照组治疗后6个月、1年复发率分别为20.59%、26.47%,治疗组复发率低于对照组(P<0.05)。结论儿制动汤加减联合生物反馈治疗具有良好的临床疗效,可以有效改善患儿中医证候评分,缓解抽动症状,减少不良反应的发生,降低复发率。Objective To explore the clinical curative effect of modified Er'zhidong Decoction combined with biofeedback on children tic disorders(TD).Methods Ninety TD children were divided into control group(Tiapride Hydrochloride tablets)and treatment group(modified Er'zhidong Decoction combined with biofeedback)by random number table method,45 cases in each group.Both groups were treated for 12 weeks.The clinical curative effect,scores of TCM syndromes and Yale Global Tic Severity Scale(YGTSS),incidence of adverse reactions and recurrence rate before and after treatment were compared between the two groups.Results The total response rates in treatment group and control group were 91.11%(41/45)and 75.56%(34/45),respectively.And the curative effect was better in treatment group than control group(P<0.05).After treatment,scores of TCM syndromes(vocal tic,exercise tic,anorexia,anxiety and irritability,mental fatigue)in both groups were significantly decreased(P<0.05),which were lower in treatment group than control group(P<0.05).After treatment,scores of vocal tic,exercise tic and comprehensive injury,and the total score of YGTSS in both groups were significantly decreased(P<0.05),which were lower in treatment group than control group(P<0.05).The incidence of adverse reactions in treatment group was lower than that in control group[4.44%(2/45)vs 20.00%(9/45)](P<0.05).After 6 months and 1 year of treatment,recurrence rates in treatment group were 4.88% and 7.32%,lower than those in control group(20.59%,26.47%)(P<0.05).Conclusion The clinical curative effect of modified Er'zhidong Decoction combined with biofeedback is good,which can effectively improve scores of TCM syndromes,relieve tics,reduce the occurrence of adverse reactions and recurrence rate.
