摘 要:目的了解山西省医疗卫生机构布鲁氏菌病(布病)诊断、报告质量,发现存在的问题,为提高布病诊断报告质量提供建议。方法采用典型抽样的方法,从山西省疫情较重的地区选择6个医疗卫生机构作为调查对象,由专业人员按照统一的方法和标准对报告的布病病例的信息质量和诊断质量进行现场调查与评价,包括查看原始登记、临床与实验室记录、现场访谈。使用Excel 2007和SPSS 16.0软件进行统计分析,分析方法采用χ2检验。结果6个机构的布病报告率为97.6%(368/377),纸质卡片填写完整率为99.5%(210/211),纸质卡片填写准确率为92.4%(194/210),网络报告信息的一致率为93.3%(196/210),网络报告及时率为94.8%(200/211)。不同机构间除纸质报告卡填写完整率的差异无统计学意义(P>0.05),其余数据报告质量指标的差异均有统计学意义(P<0.05);对6个机构338例病例的诊断质量进行评价,诊断准确率为73.1%(247/338),其中临床诊断病例和疑似病例的诊断准确率分别为22.5%(16/71)、0(0/21),不同机构间的诊断准确率差异有统计学意义(P<0.05)。结论山西省布病报告质量较好,布病报告病例的诊断准确性较低,是影响山西省布病诊断报告质量的主要问题。建议通过加强宣传,统一诊断标准,提高布病诊断准确率。Objective To understand the diagnosis of brucellosis and report quality in Shanxi Provincial medical and health organization,and find the problems,in order to promote the suggestions for brucellosis diagnosis report quality.Methods According to typical sampling survey of 6 medical and health institutions in heavy epidemic areas in Shanxi Province,field investigation and evaluation of information quality and diagnosis quality of brucellosis cases were done by professional staff according to unified methods and standards.Excel 2007 and SPSS 16.0 software were used to do statisti⁃cal analysis,and test was adpoted.Results The reporting rate of Brucellosis was 97.6%(368/377),the completed filled rate of paper cards was 99.5%(210/211),the accuracy rate of paper card filling was 92.4%(194/210),the consistency rate of network report was 93.3%(196/210),and the timely reported rate of report was 94.8%(210/211),except the completed filled rate of paper cards(P>0.05).Diagnostic accuracy rates of clinically diagnosed cases and suspected cases were 22.5%(16/71)and 0(0/21).There was statistically significant difference among different institutions(P<0.05).Conclusion The indicators of data quality related to reporting information is good,however the diagnostic accura⁃cy rate is low,which directly affects the data quality of Brucellosis in Shanxi Province.It is suggested to strengthen prop⁃organda,unify diagnosis standard,and imprve brucellosis diagnosis accuracy rate.
