摘  要:目的评估在创伤患者的现场急救处置中,采用国际创伤生命支持(International Trauma Life Support,ITLS)技术培训后的急救团队对院前急救效果的影响。方法采用回顾性分析方法,随机抽取2018-11至2018-12镇江市120调度中心接警并实施院前急救的95例创伤患者设为对照组,选取2019-01至2019-02镇江市120调度中心接警并实施院前急救的95例创伤患者设为观察组,评估接受ITLS培训的急救团队,现场停留时间如何改变,同时执行ITLS规范化操作对创伤患者的转运有何影响,有无提高患者救治成功率。结果对照组,其中男60例,女35例;年龄18~60岁,平均年龄(42.42±17.83)岁;观察组,男61例,女34例;年龄18~60岁,平均年龄(42.09±16.87)岁;对照组的现场停留时间29±9.5 min;观察组的现场停留时间19±7 min(P<0.05);对照组救治成功45例,救治成功率为49%(54/91);观察组救治成功76例,救治成功率为80%(76/95)(P<0.05);观察组与对照组相比较,通过ITLS技术团队将创伤患者分流到当地能力强的医院是非ITLS技术团队的1.7561倍(95%CL 1.0678~2.8883 P=0.027)。结论 ITLS技术可有效减少急救团队在现场的停留时间,同时影响对创伤患者的后送,可能间接提高了创伤院前急救患者救治成功率,有利于得到患者或家属对院前急救工作的认可。Objective To evaluate the effect of first aid team trained by International Trauma Life Support(International Trauma Life Support,ITLS)technology on the pre-hospital emergency treatment of trauma patients.Methods A retrospective analysis was conducted.The control group being the 95 randomly selected cases of trauma patients who called the police and was performed prehospital emergency treatment from Zhenjiang 120 dispatch center from November 2018 to December 2018.The observation group being the 95 randomly selected cases of trauma patients who called the police and was performed pre-hospital emergency treatment from Zhenjiang 120 dispatch center from January 2019 to February 2019.Evaluations were made on how ITLS team improved the dwell time and success rate of treatment,as well as what implications that standard ITLS operations had on patient transfer.Results In the control group,there were 60 males and 35 females;age ranged from 18 to 60 years old,mean age(42.42±17.83);In the observation group,there were 61 males and 34 females;age ranged from 18 to 60 years old,mean age(42.09±16.87);Dwell time was 29±9.5 minutes in the control group while 19±7 minutes in the observation group(P<0.05);45 cases were successfully treated in the control group,with a success rate of 49%(54/91);76 cases were successfully treated in the observation group,with a success rate of 80%(76/95)(P<0.05);The ITLS team was 1.7561 times higher than the non-ITLS team in the triage of trauma patients to capable local hospitals(95%confidence interval 1.0678~2.8883 P=0.027).Conclusion ITLS technology could effectively reduce the dwell time of EMS teams and influence the evacuation of trauma patients,which may indirectly improve the success rate of treating pre-hospital emergency patients and help to receive the endorsement of pre-hospital emergency work from patients and their families.
