右侧体位在低出生体重早产儿脐静脉插管术中的临床应用

摘　　要：目的:比较两种不同体位在低出生体重早产儿脐静脉插管时的应用效果。方法:选取2018年1月—2019年3月医院新生儿科收治的110例行脐静脉置管的低出生体重早产儿,采用随机数字表法分为对照组和试验组各55例。对照组采用传统方法将患儿置于仰卧位下插管,试验组将患儿维持于右侧体位下插管,比较两组患儿插管成功率、一次性插管成功率、异位至门静脉循环的发生率及置管相关并发症发生情况。结果:试验组低出生体重早产儿脐静脉插管一次性插管成功率及插管成功率均高于对照组,差异有统计学意义(P<0.05);两组尝试次数间比较,试验组尝试次数少于对照组,差异有统计学意义(P<0.05);试验组低出生体重早产儿脐静脉插管异位至门静脉循环的发生率明显低于对照组,差异有统计学意义(P<0.05)。结论:将低出生体重早产儿维持于右侧体位下进行脐静脉插管可以提高插管成功率及一次性插管成功率,降低异位至门静脉循环的发生率。Objective:To compare the effect of two different postures on umbilical vein intubation in low birth weight premature infants.Methods:A total of 110 low birth weight premature infants who underwent umbilical vein catheterization were selected from the neonatal department of hospital from January 2018 to March 2019.They were randomly divided into control group and experimental group by random number table method,with 55 cases in each group.Patients in the control group were intubated in the supine position,while those in the experimental group were intubated in the right position.The success rate of intubation,disposable intubation success rate,the incidence of ectopic to portal vein circulation and the incidence of complications related to catheterization were compared between the two groups.Results:The success rate of intubation and disposable intubation success rate in umbilical vein intubation in low birth weight premature infants in the experimental group were higher than those in the control group,and the differences were statistically significant(P<0.05).The number of attempts of umbilical vein intubation in the experimental group was lower than that in the control group,and the difference was statistically significant(P<0.05).The incidence of umbilical vein intubation to portal vein circulation in low birth weight premature infants in the experimental group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusions:The umbilical vein Intubation in low birth weight premature infants in the right side position can improve the success rate of intubation and disposable intubation success rate,and reduce the incidence of ectopic to portal vein circulation.
