摘　　要：目的探讨心内膜下心肌活力率(SEVR)对慢性心力衰竭(CHF)病人心肌能量代谢状态的评估价值。方法选取2019年2—8月安徽医科大学第一附属医院东区收治的CHF病人186例为心衰组,按照美国纽约心脏病协会(NYHA)分级分为NYHAⅡ级(n=64)、NYHAⅢ级(n=67)及NYHAⅣ级(n=55),选取2018年5月至2019年1月安徽医科大学第一附属医院健康体检者201例为健康对照组。收集两组的临床资料,包括左室射血分数(LVEF)、脑钠肽(BNP)等,利用无创心血管功能检测仪检测两组的SEVR,比较两组SEVR的差异,分析SEVR对CHF的评估价值。结果心衰组SEVR较健康对照组明显降低[(106±26)%比(129±20)%,P<0.05];SEVR与BNP呈负相关性(P<0.05);SEVR与LVEF呈正相关性(P<0.05);CHF病人随心功能分级的增高,BNP水平逐渐升高,SEVR、LVEF逐渐下降,组间两两比较均差异有统计学意义(P<0.05)。结论CHF病人SEVR水平明显降低,可作为CHF的简便易行评估手段之一。Objective To explore the evaluation value of subendocardial viability ratio(SEVR)in the assessment of myocardial energy metabolism in patients with chronic heart failure(CHF).MethodsOne hundred and eighty-six patients with CHF in East District of the First AffiliatedHospital of Anhui Medical University fromFebruary to August 2019 were selected as the study subjects.Patients were assigned into the stageⅡgroup(n=64),stageⅢgroup(n=67)and stageⅣgroup(n=55)according to the New York Heart Association(NYHA)classification.Two hundred and one healthy subjects in the First Affiliated Hospital of Anhui Medical University from May 2018 to January2019 were randomly enrolled as the control group.The correlations of the SEVR levels,the B-type natriuretic peptide(BNP)and left ventricular ejection fraction(LVEF)with chronic heart failure were analyzed.ResultsSEVR level was lower in patients with CHF than thatincontrolsubjects[(106±26)%vs.(129±20)%,P<0.05];SEVR was negatively correlated with BNP(P<0.05),SEVR was positively correlatedwithLVEF(P<0.05);BNP level was gradually increased in the stageⅡ,ⅢandⅣgroup;SEVR and LVEF levels were gradually decreased in the stageⅡ,ⅢandⅣgroup(P<0.05).ConclusionSEVR level of CHF patients is significantly decreased,which can be used as a simple and easy method to evaluate CHF.
