高明利益气养阴通络法治疗类风湿关节炎继发干燥综合征经验

摘　　要：高明利教授从事临床类风湿诊疗工作数十年,认为类风湿关节炎继发干燥综合征主要表现为气阴两虚证,其基本病机为本虚标实,本虚以气阴两虚为主,标实以瘀痹为要。据此病机,高师提出"益气养阴通络法"治疗类风湿关节炎继发干燥综合征,并基于古方四神煎加减化裁拟"益气养阴通络方"(黄芪、石斛、远志、牛膝、金银花),临证疗效显著。高师指出,"益气养阴通络法"适用于类风湿关节炎病程较长、继发干燥综合征者,以及缓解期、激素减停阶段。该文阐释本病的病机特点,总结高师"益气养阴通络法"诊疗本病的辨证思路、用药特点及临证化裁经验,本方用药特色鲜明,临床疗效确切,对类风湿关节炎继发干燥综合征的治疗有较高临床参考价值,值得学者深入学习与运用。Professor GAO has been engaging in the clinical diagnosis and treatment of rheumatoid arthritis for several decades.He has been believing that rheumatoid arthritis and secondary Sjogren’s syndrome mainly manifests the deficiency of both Qi and Yin,and the basic pathogenesis is the deficiency in origin and excess in superficislity.Among them,deficiency in origin mainly refers to the deficiency of both Qi and Yin,while enrichment in symptom refers to blood stasis.According to the pathogenesis,Professor GAO has put forward"the method of benefiting Qi and nourishing Yin and dredging collaterals"to treat rheumatoid arthritis and secondary Sjogren’ s syndrome.Based on the ancient prescription Sishen Decoction,he formulates "Yiqi Yangyin Tongluo Decoction"[Huangqi( Astragalus membranaceus),Shihu( Dendrobium),Yuanzhi( Polygala),Niuxi( Achyranthes bidentata),Jinyinhua( Lonicera japonica) ],which has a remarkable clinical effect.Professor GAO pointed out that"the method of benefiting Qi and nourishing Yin and dredging collaterals"is suitable for the patients with long course and secondary Sjogren’ s syndrome,patients in the remission period of rheumatoid arthritis and the process of hormones eduction and cessation.This article explained the pathogenesis of this disease and summarized ideas of syndrome differentiation to disease treatment,characteristics of medication and experience in clinical modification of"the method of benefiting Qi and nourishing Yin and dredging collaterals"from Professor GAO.This prescription has distinct medication characteristics and definite curative effect,and so it has high clinical reference value for rheumatoid arthritis and Secondary Sjogren’ s syndrome.It is also worthy of further study and popularization.
