伊血安颗粒与新生化冲剂治疗产后恶露不绝的临床随机对照研究

摘　　要：目的探讨伊血安颗粒与新生化冲剂治疗产后恶露不绝的临床随机对照研究。方法将2017年4月—2018年8月该院诊治的74例血瘀型产后恶露不绝的患者随机分为伊血安组和新生化组,每组37例。两组均给予缩宫、抗感染等基础综合治疗,伊血安组给予伊血安颗粒15 g/次,3次/d;新生化组给予新生化颗粒12 g/次,3次/d。两组均持续治疗7 d。详细记录平均止血时间,检测血液流变学:血浆黏度、全血切度、红细胞积压、纤维蛋白原,评价中医证候积分,比较临床疗效。结果与治疗前比较,两组血浆黏度、全血低、高切度、红细胞积压、纤维蛋白原降低(P <0.01),阴道出血、腹痛、面色证候积分降低(P <0.01);与伊血安组比较,新生化组平均止血时间无统计学意义(P> 0.05),血浆黏度、全血低、高切度、红细胞积压、纤维蛋白原无统计学意义(P> 0.05),阴道出血、腹痛、面色证候积分无统计学意义(P> 0.05),总有效率无统计学意义(P> 0.05)。结论伊血安颗粒与新生化颗粒治疗血瘀型产后恶露不绝临床疗效相当,值得临床推广。Objective To investigate the clinical randomized controlled study of Yixue’an Granule and Xinshenghua Granule in the treatment of postpartum lochia.Methods Seventy-four patients with postpartum lochia of blood stasis type from April 2017 to August 2018 were randomly divided into Yixue’ an Granule group and Xinshenghua Granule group,37 cases in each group.Two groups were given basic comprehensive treatment such as uterine contraction and anti-infection,Yixue’an Granule group was given Yixue’an Granule 15 g,three times a day,and Xinshenghua Granule group was given Xinshenghua Granule 12 g,three times a day,and two groups were treated for 7 days.The average hemostasis time was recorded in detail,and the hemorheology,such as plasma viscosity,whole blood shear,hematocrit and fibrinogen,were measured.The scores of TCM symptoms were evaluated and the clinical effects were compared.Results Compared with those before treatment,plasma viscosity,whole blood low,high shear,hematocrit and fibrinogen obviously decreased( P < 0.01),vaginal bleeding,abdominal pain and facial color syndrome decreased( P < 0.01).Compared with those of the Yixue’an Granule group,the average hemostasis time of Xinshenghua Granule group had no statistical significance( P > 0.05),plasma viscosity,whole blood low,high shear,hematocrit and fibrinogen had no lineage( P > 0.05).There was no significant difference in the scores of vaginal bleeding,abdominal pain or facial symptoms( P > 0.05),and there was no significant difference in the total effective rate( P > 0.05).Conclusion Yixue’ an Granule and Xinshenghua Granule have the same clinical effect in the treatment of postpartum lochia of blood stasis type,which are worthy of clinical promotion.
