119例系统性红斑狼疮患者心电图分析

摘　　要：目的分析系统性红斑狼疮(systemic lupus erythematosus,SLE)异常心电图的表现内容,并归纳总结异常心电图的临床意义。方法收集2013年10月至2018年1月本院就诊的119例SLE患者的心电图资料,并结合病例档案进行回顾性分析,其中心脏异常患者59例,心脏正常患者60例,分别作为心电异常组和心电正常组。结果分析结果显示,119例被确诊系统性红斑狼疮患者中,经心电图检查一共发现59例有心电图异常,其中以心律失常患者所占比例最大,共33例,占心电图异常患者55.93%(33/59),ST-T改变异常患者占据36例,占心电图异常患者44.07%(26/59),心电异常组患者的病程时间平均为(21.93±7.45)月,心电正常组患者的病程时间平均为(12.36±4.71)月,差异具有统计学意义(P<0.05)。系统性红斑狼疮患者中出现95例肺动脉高压,其中心电异常患者中有51例出现肺动脉高压,占比86.44%(51/59),心电正常组患者中合并有44例患者出现肺动脉高压,占比73.33%(44/60),差异具有统计学意义(P<0.05),且心电异常患者的超声心脏各项功能较心电正常组患者检查结果较差,差异具有统计学意义(P<0.05)。结论系统性红斑狼疮患者的心电图出现异常的概率较高,因此通过心电图检查了解心脏受损程度。通过心电图还能指导冠状动脉造影检查,及时诊断和治疗心脏损害,对预防并发症发生具有积极意义。Objective To analyze the ECG manifestations of systemic lupus erythematosus(SLE)and summarize the clinical significance.Methods The electrocardiogram data of 119 SLE patients admitted to our hospital from October 2013 to January 2018 were collected and retrospectively analyzed based on the case files.Among them,59 patients with abnormal heart and 60 patients with normal heart were selected as the electrocardiogram abnormal group and the electrocardiogram normal group,respectively.Results The results showed that among 119 patients with SLE,59 cases were found to have abnormal ECG.Among them,33 cases were arrhythmia,accounting for 55.93%(33/59)of patients with abnormal ECG,36 cases with abnormal ST-T changes,accounting for 44.07%(26/59)of patients with abnormal ECG The average duration was(21.93±7.45)months,and the average course time of patients in normal ECG group was(12.36±4.71)months,the difference was statistically significant(P<0.05).Pulmonary hypertension was found in 95 patients with systemic lupus erythematosus.Among them,51 patients with abnormal ECG had pulmonary hypertension,accounting for 86.44%(51/59).In patients with normal ECG,44 patients had pulmonary hypertension,accounting for 73.33%(44/60).The difference was statistically significant(P<0.05),and the ultrasonic cardiac function of patients with abnormal ECG was higher than that of normal ECG group The examination results of patients were poor,the difference was statistically significant(P<0.05).Conclusions The probability of abnormal ECG in patients with systemic lupus erythematosus is high,so we can know the degree of heart damage by ECG examination.ECG can also guide coronary angiography,timely diagnosis and treatment of heart damage,it can prevent complications which have a positive significance.
