摘　　要：目的通过对全院手术科室非计划再次手术的发生原因及影响因素进行分析,为提高手术质量、保障医疗安全和质量提供管理依据。方法回顾性分析某三级甲等肿瘤专科医院2015-2019年272例非计划再次手术病例,对非计划再次手术的发生原因进行分析。结果2015-2019年发生非计划再次手术272例,发生率为2.51‰;非计划再次手术男性177例,占65.1%;60岁及以上年龄组有121例,占44.48%;非计划再次手术主要发生在神经外科(23例,发生率26.35‰)、胃肠外科(81例,发生率18.67%)和泌尿外科(39例,发生率13.44‰);术后出血,术后切口裂开、肠漏为非计划再次手术发生的主要原因,占总原因的60.67%。结论严格落实手术手术分级管理制度,重点科室重点管理,加强医师手术操作培训,提高医疗技术水平,降低非计划再次手术发生率。Objective To analyze the reasons and influencing factors of unplanned reoperation,and provide the management basis for improving the quality of operation and ensuring medical safety.Methods A retrospective analysis was carried out in 272 unplanned reoperation cases from 2015 to 2019 in a third-grade tumor hospital,and the incidence of unplanned reoperations was analyzed.Results There were 272 cases of unplanned reoperation from 2015 to 2019,the incidence of unplanned reoperation was 2.51‰;It mainly happened in males(177 cases,65.1%);Ages over 60 were 121 cases,accounting for 44.48%;The reoperated patients were mainly distributed at the department of neurosurgery(23 cases,26.35‰),gastrointestinal surgery(81 cases,18.67‰)and urology surgery(39 cases,13.44‰);The main causes of unplanned reoperation included postoperative bleeding,postoperative incision dehiscence and intestinal leakage(60.67%).Conclusions The grading management system of surgical operations should be strictly implemented,and the training of doctors in surgical operations should be strengthened too.Important departments should be managed especially,so that medical technology could be improved,and the incidence of unplanned reoperation could reduced.
