舒适护理及疼痛管理对肛裂术后的干预研究

摘　　要：目的研究比较舒适护理联合疼痛管理对肛裂手术治疗患者伤口愈合情况、术后VAS评分的影响。方法选取本院2017年3月—2018年3月择期实施肛裂手术患者108例,按惯序标号随机分组,对照组54例,实施综合护理干预,观察组54例,视肛裂伤痛特征给予疼痛管理并开展舒适护理,评估两组术后VAS评分,伤口愈合效果、Wexner便秘评分、护理满意度及负性情绪评分。结果观察组术后1-5dVAS评分低于对照组(P<0.05),观察组住院时间、Wexner便秘评分低于对照组(P<0.05),满意度评分高于对照组(P<0.05),两组下床时间、创口愈合时间差异无统计学意义(P>0.05),观察组术后5dHAMA、HAMD评分均明显低于对照组(P>0.05)。结论肛裂术后患者开展舒适护理配合疼痛管理进行服务效果良好,能有效减轻肛裂疼痛,改善心理负性情绪,促进创口愈合,提高服务满意率。Objective To study the effect of comfortable nursing combined with pain management on wound healing and postoperative VAS score in patients with anal fissure.Methods From March 2017 to March 2018,108 patients undergoing anal fissure surgery were selected.54 patients in the control group were randomly divided into two groups:the control group(n=54)received comprehensive nursing intervention,and the observation group(n=54)were treated with pain management and comfort nursing according to the characteristics of anal fissure injury.The VAS score and the score of wound healing were evaluated in the two groups,and the effect of wound healing was evaluated by Wexner constipation score.Results The 1-5dVAS score of the observation group was significantly lower than that of the control group(P<0.05),the hospitalization time of the observation group was significantly lower than that of the control group(P<0.05),the satisfaction score of the observation group was significantly higher than that of the control group(P<0.05),and the time of getting out of bed was significantly higher in the observation group than in the control group(P<0.05).There was no significant difference in wound healing time(P>0.05),and the scores of Hamah Hamd in the observation group were significantly lower than those in the control group on the 5th day after operation(P>0.05).Conclusion The comfortable nursing and pain management of the patients after anal fissure have a good effect and can effectively reduce the pain of anal fissure.Improve psychological negative emotion,promote wound healing,improve service satisfaction rate.
