宫内节育器嵌顿的高危因素分析

摘　　要：目的探讨宫内节育器(IUD)嵌顿的高危因素及预防措施。方法选取2017年1月至2019年8月本院收治112例IUD嵌顿患者为观察组,同期未发生IUD嵌顿的96例女性为对照组。比较两组一般情况、病史、IUD使用史等临床资料,采用多元逐步Logistic回归分析IUD嵌顿的危险因素。结果节育器种类、痛经、妇科炎症、带环时间、带环怀孕均为导致IUD嵌顿的独立危险因素(P<0.05)。结论IUD嵌顿发生与多种因素密切相关,放置IUD时应考虑妇女综合状态,选择合适的IUD种类,加强术后观察和随访,及时治疗妇科炎症,结合IUD的使用年限及时更换,以减少嵌顿情况发生。Objective To explore the high-risk factors and preventive measures for incarceration of intrauterine device(IUD).Methods 112 patients with IUD incarceration admitted to our hospital from January 2017 to August 2019 were selected as observation group,and 96 women without IUD incarceration during the same period were selected as control group.The general status,medical history,history of IUD use and other clinical data were compared between the two groups,and the risk factors of IUD incarceration were analyzed by multivariate stepwise Logistic regression analysis.Results The types of IUDs,dysmenorrhea,gynecological inflammation,IUD carrying time and pregnancy during IUD were all independent risk factors for IUD incarceration(P<0.05).Conclusion The occurrence of IUD incarceration is closely related to many factors.When placing the IUD,it is necessary to consider the overall status of women to select appropriate IUD type,strengthen the postoperative observation and follow-up,treat the gynecological inflammation in time,and replace the IUD in time according to its use years so as to reduce incarceration.
