摘  要:目的探讨分段读出长可变回波序列(Readout Segmentation of Long Variable Echo Trains,RESOLVE)在脑肿瘤高场强术中磁共振扩散加权成像(Diffusion-Weighted Imaging,DWI)中的应用价值。方法采用双室移动磁体高场强磁共振成像系统,对21例胶质瘤或脑膜瘤患者在颅内肿瘤切除后颅骨缝合前行T1WI、常规DWI和RESOLVE-DWI序列扫描。由两名影像医生对两组DWI图像质量进行主观评分,评估内容包括图像变形程度、空间分辨率和病灶手术区域细节显示,并定量评估表观扩散系数(Apparent Diffusion Coefficient,ADC)值的差异及手术区域的信噪比(Signal Noise Ratio,SNR)。采用配对t检验方法比较两组DWI序列的图像质量评分、SNR和ADC值。结果RESOLVE-DWI的图像质量评分、SNR值均高于常规EPIDWI图像,差异有统计学意义(P<0.001),但两组DWI图像的ADC值无统计学差异(P>0.05)。结论RESOLVE-DWI技术可以有效提高脑肿瘤术中DWI的图像质量,减轻由于颅内植入物以及手术床头架钉对DWI图像造成的伪影和变形程度。Objective To explore the application value of high-resolution diffusion-weighted imaging(DWI)of readout segmentation of long variable echo trains(RESOLVE)in high-field intraoperation magnetic resonance imaging of brain tumor.Methods T1 WI,conventional DWI and RESOLVE-DWI sequences of 21 patients with glioma or meningioma were scanned by high-field intraoperation MRI system during surgery.Two radiologists evaluated and scored the quality of the images,including the anatomic distortion,spatial resolution,and surgical area conspicuity.The signal noise ratio(SNR)of the operation area and difference of apparent diffusion coefficient(ADC)of were evaluated quantitatively.The paired t-test was used to compare the image quality score,SNR and ADC value of the two DWI sequences.Results The image quality score and SNR of RESOLVE-DWI were higher than those of conventional EPI-DWI,the difference was statistically significant(P<0.001),but there was no significant difference in ADC value of DWI between the two groups(P>0.05).Conclusion RESOLVE-DWI technology can effectively improve the image quality of DWI in brain tumor surgery,and reduce artifacts and distortion of DWI image.
