摘 要:目的:探讨NSMR对常规体外受精-胚胎移植(IVF-ET)临床治疗结局的影响。方法:回顾性分析在本中心行IVF治疗的565个取卵周期,根据WHO 5版推荐的Diff-Quick染色方法及评价标准对男方精液进行形态学分析,研究分为3组:A组:中度畸形组,1%≤NSMR<2.5%;B组:轻度畸形组,2.5%≤NSMR<4%;正常形态组:NSMR≥4%。分别比较三组间IVF受精率、卵裂率、优质胚胎率、可利用胚胎率、囊胚形成率等指标有无显著性差异。结果:本次研究的三组患者一般资料(不孕年限、女方年龄、基础FSH、BMI指数、获卵总数、MII卵子数)间均无统计学差异(P>0.05)。三组间2PN受精率、卵裂率、可利用胚胎率、优质胚胎率、囊胚形成率等均无统计学差异(P>0.05),受精率、多精受精率随着NSMR升高而升高,三组间差异具有显著性(P<0.05)。结论:在常规的IVF治疗过程中,NSMR对受精率有一定的影响,但对IVF的胚胎发育质量方面无明显预测性。Objective:To explore the influence of normal sperm morphology ratio(NSMR)on the clinical outcomes in routine in vitro fertilization(IVF).Methods:This study retrospectively analyzed 565 IVF cycles.Sperm morphology was evaluated by Diff-Quick staining methods according to the WHO 5 criterion.Based on the results of morphological evaluation,the 565 cycles were divided into three groups,which defined NSMR≥1%and<2.5%as group A,≥2.5%and<4%as group B,and≥4%as group C.The relationship between sperm morphology and the rates of fertilization,cleavage,available embryo,high-quality embryo,and blastocyst formation were compared and analyzed among these three groups.Results:There was no significant difference among the three groups in female age,infertile age,BMI,base FSH level,oocyte number and MII oocyte number(P>0.05).No statistically significant difference were observed in the rates of 2 PN-fertilization,cleavage,available embryo,high-quality embryo,and blastocyst formation(P>0.05).Meanwhile,there was statistically significant difference in the rate of fertilization and multi-PN fertilization among the three groups(P<0.05),following with increase of NSMR,the fertilization and multi-PN fertilization were significantly increased.Conclusion:NSMR can affect the rate of fertilization,however,has no significant predictive value on embryo quality in IVF.
