侧卧斜俯截石位经皮肾镜碎石取石术与腹腔镜输尿管切开取石术治疗输尿管中段结石临床比较

摘　　要：目的:探究侧卧斜俯截石位经皮肾镜碎石取石术与腹腔镜输尿管切开取石术治疗输尿管中段结石的临床效果差异。方法:选取2017年8月—2019年4月在我院进行治疗的81例输尿管中段结石患者,按随机数字表法将其分成2组,对照组40例患者给予腹腔镜输尿管切开取石术治疗,观察组41例患者给予侧卧斜俯截石位经皮肾镜碎石取石术治疗。对比两组患者术中及术后临床指标。结果:观察组患者术后收缩压、心率、手术时间、术中出血量、住院时间、术后血尿时间和并发症发生率明显优于对照组患者,差异具有统计学意义(P<0.05)。结论:侧卧斜俯截石位经皮肾镜碎石取石术可以有效稳定输尿管中段结石患者术中收缩压和心率,缩短手术时间、住院时间、术后血尿时间,减少术中出血量和并发症发生率。Objective:To explore the clinical effect difference between percutaneous nephrolithotomy and laparoscopic ureterolithotomy in the treatment of middle ureteral calculi.Methods:81 patients with middle ureteral calculi treated in our hospital from August,2017 to April,2019 were randomly divided into two groups.40 patients in the control group were treated with laparoscopic ureterolithotomy,while 41 patients in the observation group were treated with percutaneous nephrolithotomy in the lateral oblique lithotomy position.The clinical indexes during and after operation were compared between the two groups.Results:Postoperative systolic blood pressure,heart rate,operation time,intraoperative blood loss,hospitalization time,postoperative hematuria time and complication rate of the observation group were significantly better than those of the control group(P<0.05).Conclusion:Percutaneous nephrolithotomy in lateral oblique lithotripsy position can effectively stabilize systolic blood pressure and heart rate,shorten operation time,hospitalization time,postoperative hematuria time,and reduce intraoperative blood loss and complication rate.
