经直肠彩色多普勒超声(TRS)在功能性子宫出血诊断中的应用价值

摘　　要：目的:分析经直肠彩色多普勒超声(TRS)在功能性子宫出血诊断中的应用价值。方法:选取68例因规则阴道出血、月经过多或月经过长就诊的女性,分别实施经腹部、经直肠彩色多普勒超声检查,此后行刮宫病理检查,对经腹部、经直肠彩超的诊断结果进行判断。结果:68例患者病理诊断为功能性子宫出血46例,经腹部超声检查的敏感度、特异度、准确度为78.26%、62.50%、76.47%,经直肠彩超检查的敏感度、特异度、准确度为93.48%、87.50%、94.12%,两种方法比较各指标差异显著(P<0.05)。总结彩超检查结果,功能性子宫出血患者子宫形态基本规则或规则,子宫内膜回声强或较强,部分可测及小卵泡,血流频谱检查结果显示,S/D为(2.10±0.18),RI为(0.52±0.07)。结论:经直肠彩色多普勒超声在功能性子宫出血诊断中应用价值显著,可为患者疾病治疗提供重要参考信息,值得推广。Objective:To analyze the application value of Transrectal Color Doppler Ultrasound(TRS)in the diagnosis of functional uterine bleeding.Methods:68 women with regular vaginal bleeding,menorrhagia or long menstrual period were selected to receive transabdominal and transrectal color Doppler ultrasonography,and then curettage pathological examination was performed to judge the diagnosis results of transabdominal and Transrectal Color Doppler Ultrasound.Results:46 of the 68 patients were pathologically diagnosed as functional uterine bleeding.The sensitivity,specificity and accuracy of transabdominal ultrasound examination were 78.26%,62.50%and 76.47%,and the sensitivity,specificity and accuracy of transrectal ultrasound examination were 93.48%,87.50%and94.12%,respectively.There were significant differences between the two methods(P<0.05).To sum up the results of color ultrasound examination,the basic rules or rules of uterine morphology of patients with functional uterine bleeding,strong or strong endometrial echo,some of which can be measured and small follicles,and the results of blood flow spectrum examination showed that S/D was(2.10±0.18)and RI was(0.52±0.07).Conclusion:Transrectal color Doppler ultrasonography has significant value in the diagnosis of functional uterine bleeding,which can provide important reference information for the treatment of patients with disease and is worthy of promotion.
