摘　　要：目的分析经皮针刺筋膜切开术和脂肪填充对女性因气性坏疽导致臀骶部感染手术术后凹陷性瘢痕的美学矫正效果。方法回顾性分析与我院采用经皮针刺筋膜切开术和脂肪填充治疗的1例因气性坏疽导致臀骶部感染手术术后凹陷性瘢痕的女性患者的临床资料,观察其治疗效果。结果第一阶段自体脂肪移植瘢痕修复手术治疗+脂肪移植丰臀术后,臀区脂肪注射量为600 ml,其中双侧臀部各移植脂肪300 ml,无严重不良反应发生。术后7 d~21个月随访期间,脂肪来源干细胞的再生效应逐渐显现,臀区碟状凹陷性瘢痕改善,臀区皮下组织缺损及其区域的低陷恢复正常,臀区轮廓外形恢复正常,臀区皮肤平整度提升。第二阶段自体脂肪移植瘢痕修复手术治疗+脂肪移植丰臀术后,臀区脂肪注射量为600 ml,其中双侧臀部各移植脂肪300 ml,无严重不良反应发生。术后7 d~21个月随访期间,臀区线状增生性瘢痕改善,皮肤恢复平整度、柔软度、弹性进一步增强;臀型整体凸度提升,外观圆润丰满,腰臀比达到美学要求,腰臀腿美学轮廓线自然过渡衔接,臀区轮廓与整体形体轮廓协调美观。结论经皮针刺筋膜切开术结合脂肪填充治疗臀骶部感染手术术后瘢痕畸形,同时结合脂肪移植丰臀术、吸脂术可获得良好的美化效果,受术者满意度高;脂肪移植手术填充部位皮肤平整,外观自然,弹性好;原有皮肤瘢痕消退效果肯定,达到就诊者对美容的期望值,其成功的关键是彻底释放纤维化粘连。Objective To analyze the aesthetic correction effect of percutaneous acupuncture fasciotomy and fat filling in depressed scar after operation of buttock and sacral infection caused by gas gangrene in women Methods The clinical data of a female patient with depressed scar after operation of buttock and sacral infection caused by gas gangrene was retrospectively analyzed and the therapeutic effect was observed.Results After the first stage of autologous fat transplantation scar repair surgery+fat transplantation buttock augmentation operation,the fat injection volume in buttock area was 600 ml,including 300 ml fat transplantation in bilateral buttocks,and no serious adverse reactions occurred.During the follow-up period from 7 days to 21 months after operation,the regeneration effect of adipose derived stem cells gradually appeared,the dished concave scar of buttock area was improved,the subcutaneous tissue defect and the depression of buttock area returned to normal,the contour of buttock area returned to normal,and the skin smoothness of buttock area was improved.After the second stage of autologous fat transplantation scar repair surgery+fat transplantation buttock augmentation surgery,the amount of fat injection in buttock area was 600 ml,including 300 ml fat transplantation in bilateral buttocks,and no serious adverse reactions occurred.During the follow-up period from 7 days to 21 months after operation,the linear hypertrophic scar of buttock area was improved,and the skin was restored with smoothness,softness and elasticity further enhanced.The overall convexity of hip shape was improved,the appearance was round and plump,the waist hip ratio met the aesthetic requirements,the aesthetic contour line of waist hip leg was naturally transitional and connected,and the hip area contour was harmonious and beautiful with the overall body contour.Conclu sion Percutaneous acupuncture fasciotomy combined with fat filling in the treatment of depressed scar after operation treatment of buttocks and sacral infections,comb
