通脉养心丸联合β受体阻断剂治疗冠心病心律失常效果研究

摘　　要：目的探讨通脉养心丸联合β受体阻断剂治疗冠心病心律失常的效果。方法选择2015年1月至2018年1月心内科治疗的冠心病心律失常患者120例随机分为对照组和观察组,每组60例。对照组使用常规冠心病二级预防药物治疗(抗血小板、他汀类药物、硝酸酯类药物以及β受体阻断剂),观察组在此基础上使用通脉养心丸,比较2组的临床疗效、中医证候积分变化、心电图指标变化、不良反应。结果观察组治疗有效率为95.16%明显高于对照组的75.81%(P<0.05);2组治疗前中医证候积分差异无统计学意义(P>0.05),观察组治疗后中医证候积分明显低于对照组(P<0.05);2组治疗前期前收缩及次数和短阵室速次数差异无统计学意义(P>0.05),观察组治疗后期前收缩及次数和短阵室速次数明显少于对照组(P<0.05);2组治疗前ST段压低持续时间、QTc间期、QTc离散度差异无统计学意义(P>0.05),观察组治疗后ST段压低持续时间、QTc间期、QTc离散度均明显短于对照组(P<0.05);2组窦性心动过缓、皮疹、恶心呕吐、视物模糊、肝肾功能异常等不良反应发生率差异无统计学意义(P>0.05)。结论通脉养心丸联合β受体阻断剂治疗冠心病心律失常能有效改善心律,抑制窦房结自律性,改善心脏电生理特性,延长动作电位,同时增加缺血心肌的供血供氧,保护心功能,有助于抑制病情的进展,具有积极的临床意义。Objective To investigate the therapeutic effects of Tongmaiyangxin pill combined with beta blocker on coronary heart disease(CHD)arrhythmia.Methods A total of 120 patients with CHD arrhythmia who were treated in our hospitalk from January 2015 to January 2018 were enrolled in the study,who were randomly divided into control group and observation group,with 60cases in each group.The patients in control group were treated by conventional secondary prevention durgs of coronary heart disease,including antiplatelet,statins,nitrates,and beta blockers,however,the patients in observation group,on the basis of control group,were treated by Tongmaiyangxin pill.The clinical curative effects,and the changes of TCM syndrome scores,electrocardiogram(ECG)and adverse reactions were observed and compared between the two groups.Results The effective rate in observation group was 95.16%,which was significantly higher than that(75.81%)in control group(P<0.05).Before treatment,there were no significant differences in TCM syndrome scores of palpitation,chest tightness,shortness of breath,mental fatigue and fatigue,lip nail dimness between the two groups(P>0.05).After treatment,the TCM syndrome scores of palpitation,chest tightness,shortness of breath and fatigue,mental fatigue and fatigue,lip nail dimness in observation group were significantly lower than those in control group(P<0.05).Before treatment there were no significant differences in the frequency of premature systole and the number of times of short burst ventricular tachycardia between the two groups(P>0.05).After treatment the frequency of premature systole and the number of times of short burst ventricular tachycardia in observation group were significantly less than those in control group(P<0.05).Before treatment there were no significant differences in ST segment depression duration,QTc interval and QTc dispersion between two groups(P>0.05),however,after treatment the ST segment depression duration,QTc interval and QTc dispersion in observation group were significantly
