腰椎后路滑脱复位手术中曲度矫正的手术配合及意义

摘　　要：目的探讨腰椎后路滑脱复位椎间植骨融合内固定术中G型臂透视下曲度矫正对术后融合率及临床疗效的影响。方法选取2011年7月至2016年7月治疗的腰椎滑脱患者188例,其中男72例,女116例。Ⅰ度滑脱68例,Ⅱ度滑脱88例,Ⅲ度滑脱32例。所有患者均行后路椎弓根螺钉固定、复位、椎间融合术。对照组腰椎滑脱症患者96例,男31例,女65例,行常规腰后路滑脱复位内固定手术。观察组患者92例,男36例,女56例,腰椎后路滑脱复位椎间植骨融合内固定术中应用G型臂透视下进行曲度矫正。所有患者术前及术后行X线及CT检查,对2组患者基本情况、临床效果、融合率、滑脱复位程度、椎间隙高度、内固定系统松动及断裂情况进行比较。结果术后随访26~52个月,平均36个月。2组患者在术前性别比、年龄、滑脱程度上差异无统计学意义(P>0.05)。2组患者术后临床评分改善比较有统计学意义(P<0.05)。2组患者术后融合率比较有统计学意义(P<0.05);椎间隙高度丢失统计学意义显著(P<0.05);术后腰椎滑脱恢复程度比较无统计学意义(P>0.05);最终随访时滑脱复位后再前移位情况比较有统计学意义(P<0.05)。对照组术后1年随访出现内固定松动1例。结论腰椎后路滑脱复位椎间植骨融合内固定术中G型臂透视下曲度矫正对术后融合率及临床疗效的影响。Objective To investigate the effects of G-type arm fluoroscopic curvature correction in the lumbar posterolateral spondylolisthesis restoration intervertebral bone graft fusion internal fixation on postoperative fusion rate and the clinical effects.Methods A total of 188 patients with lumbar spondylolisthesis who were treated in our hospital from July 2011 to July 2016 were enrolled in the study,including 72 males and 116 females,there were 68 cases ofⅠdegree spondylolisthesis,88 cases ofⅡdegree spondylolisthesis,32 cases ofⅢdegree spondylolisthesis.All the patients were underwent posterior pedicle screw fixation,restoration and interbody fusion.The 96 patients with lumbar spondylolisthesis in control group were treated by routine posterior lumbar restoration and internal fixation,including 31 males and 65 females.The 92 patients in observation group were treated by posterior lumbar spondylolisthesis restoration,interbody fusion and internal fixation with G-type arm fluoroscopic curvature correction,including 36 males and 56 females.All the patients were examined by X-ray and CT examination before and after operation.The basic condition,clinical effects,fusion rate,and restoration degree,height of intervertebral space,loosening and rupture of internal fixation system were observed and compared between the two groups.Results All patients were followed up for 26~52 months after operation,with average time being 36 months.Before operation there were no significant differences in patient’s gender and age,and spondylolisthesis degree between the two groups(P>0.05).After operation there were significant differences in clinical symptom improvement scores between the two groups(P<0.05),especially in the intervertebral space height loss(P<0.05).After operation there was no significant difference in the recovery degree of lumbar spondylolisthesis between the two groups(P<0.05),however,there was significant difference in the antedisplacement status after spondylolisthesis restoration at the final follow-up between t
